乳腺导管内肿物完形抓取活检对比单导管腺叶切除术临床分析

摘　　要：目的探讨完形抓取活检系统对乳管内单发肿物的治疗效果。方法回顾性分析2020年6月至2021年6月于西安交通大学第一附属医院乳腺外科行病灶完形抓取活检术和单导管腺叶切除术的单发导管内乳头状瘤各15例。分析两组患者的病理检出率,以及伤口长度、换药次数、住院时间、伤口愈合时间的临床恢复指标。结果病灶完形抓取活检术组的病理检出率为100.0%,高于单导管腺叶切除术组的86.7%,但差异无统计学意义(P>0.05)。病灶完形抓取活检术组患者的伤口长度为(0.85±0.23)cm,小于单导管腺叶切除术的(2.97±0.77)cm;病灶完形抓取活检术组的换药次数为(1.40±0.51)次、住院时间为(1.20±0.41)天、伤口愈合时间为(5.47±0.64)天,均短于单导管腺叶切除术组的(3.20±0.56)次、(2.87±0.64)天、(8.80±1.21)天,差异均有统计学意义(t值分别为10.225、9.222、8.469、9.449,P<0.01)。结论乳腺病灶完形抓取技术可一次性完整切除乳腺导管内肿物,提高了病理检出率,促进患者临床恢复,从而为诊断提供了更可靠的依据,为治疗提供了更安全的措施。Objective To explore the therapeutic effect of intact excision biopsy system on single tumor in breast duct.Methods A retrospective analysis of patients of single intraductal papilloma who underwent intact lesion excision boipsy and single duct breast lobectomy in the department of breast surgery of the First Affiliated Hospital of Xi′an Jiaotong University from June 2020 to June 2021,with 15 cases for each method was conducted.The pathological detection rates and clinical recovery indicators including wound length,number of dressing changes,hospitalization time,and wound healing time of the two groups′patients were analyzed.Results The pathological detection rate of intact lesion excision boipsy group was 100%,which was higher than 86.7%of the single duct breast lobectomy group,but the difference was not statistically significant(P>0.05).The wound length of intact lesion excision boipsy group was(0.85±0.23)cm,which was less than(2.97±0.77)cm of single duct breast lobectomy.In the intact lesion excision boipsy group,the number of dressing changes was(1.40±0.51)times,hospitalization time was(1.20±0.41)days,wound healing time was(5.47±0.64)days,all were shorter than those of the single duct breast lobectomy group with(3.20±0.56)times,(2.87±0.64)days and(8.80±1.21)days,and the differences were statistically significant(t=10.225,9.222,8.469 and 9.449,respectively,P<0.01).Conclusion Intact breast lesion excision boipsy technology could completely remove the tumor in breast duct at one time,improve the pathological detection rate,and promote the clinical recovery of patients,so as to provide a more reliable basis for diagnosis and safer measures for treatment.
