彩超引导下经皮穿刺腹膜透析置管术的临床应用

摘　　要：目的 观察彩超引导下经皮穿刺腹膜透析置管术的临床效果。方法 选取江门市中心医院120例新置管腹膜透析患者,对其临床资料进行回顾性研究。120例患者中,50例于切开直视下行腹膜透析置管术(对照组),其余70例均于彩超引导下行经皮穿刺腹膜透析置管处理(观察组),对比组间临床效果。结果 观察组操作时间、伤口长度、术后第一天疼痛评分显低于对照组,2组术中均未出现明显出血,置管术后1个月内相关合并症、感染发生率差异均无统计学意义(P>0.05)。结论 临床上,肾脏内科医生比较容易掌握彩超引导下经皮穿刺腹膜透析置管术,并且该术式不会对患者造成较大创伤,其并发症的发生也比较容易控制。Objective To observe clinical effect of percutaneous peritoneal dialysis catheterization guided by color Doppler ultrasound.Methods The paper chose120 cases new catheter peritoneal dialysis patients in our peritoneal dialysis center,studied and reviewed their clinical data.Among the 120 cases,50 cases were treated with peritoneal dialysis catheterization under open vision (control group),the other 70 cases with percutaneous peritoneal dialysis catheterization under guidance of color Doppler ultrasound (observation group).Clinical effect between two groups was compared.Results Operation time,wound length and pain scores on the first day after operation in observation group were significantly lower than control group.There was no significant bleeding in two groups,no significant difference in incidence of related complications and infection within one month after catheterization (P>0.05).Conclusion Clinically,it is easy for nephrologists to master percutaneous peritoneal dialysis catheterization under guidance of color Doppler ultrasound,which may not cause great trauma to patients,and complication is easy to control.
