摘　　要：目的探讨2种前列腺剜除术结合中医传统治疗前列腺增生的临床疗效。方法将66例前列腺增生患者随机分为两组(各33例),对照组行经尿道等离子前列腺剜除术,试验组行经尿道钬激光前列腺剜除术,术后均施行耳穴贴压及腕踝针治疗。比较两组手术指标、最大尿流率、残余尿量、国际前列腺症状评分(IPSS)及并发症发生率。结果两组手术时间无显著差异(P>0.05);但与对照组相比,试验组导尿管留置时间、住院时间更短,出血量更少(P<0.05)。治疗后,两组最大尿流率提高,残余尿量减少,但组间比较无明显差异(P>0.05),较对照组,试验组IPSS更低(P<0.05)。试验组血尿、尿道狭窄、短暂性尿失禁等并发症发生率较对照组更低(P<0.05)。结论结合中医传统治疗的前列腺剜除术,钬激光前列腺剜除术比等离子前列腺剜除术的出血量更少,术后康复时间更短,可更好的改善前列腺症状,减少术后并发症,建议临床应予以优先选择。Objective To explore the clinical efficacy of two kinds of enucleation of prostate combined with traditional Chinese medicine in the treatment of benign prostatic hyperplasia.Methods A total of 66 patients with benign prostatic hyperplasia were randomly divided into two groups(33 cases in each group).The control group underwent transurethral plasma enucleation of the prostate and the experimental group underwent transurethral holmium laser enucleation of the prostate.All patients were treated with auricular point sticking and wrist ankle acupuncture.The operative indexes,maximum urinary flow rate,residual urine volume,International Prostate Symptom Score(IPSS)and the incidence of complications were compared between the two groups.Results There was no significant difference in operation time between the two groups(P>0.05),but the indwelling time and hospitalization time of urinary catheter in the experimental group were shorter and the amount of bleeding was less(P<0.05).After treatment,the maximum urinary flow rate increased and the residual urine volume decreased in the two groups,but there was no significant difference between the two groups(P>0.05).Compared with the control group,the experimental group was lower IPSS(P<0.05).The incidence of complications such as hematuria,urethral stricture and transient urinary incontinence in the experimental group was lower than that in the control group(P<0.05).Conclusion Combined with traditional Chinese medicine treatment of enucleation of prostate,holmium laser enucleation of prostate has less bleeding and shorter postoperative rehabilitation time than plasma enucleation of prostate,which can better improve prostate symptoms and reduce postoperative complications.It is recommended that clinical selection should be given priority.
