静脉用丙种球蛋白治疗新生儿ABO溶血病的临床分析

摘  要:目的:分析静脉丙种球蛋白治疗新生儿ABO溶血症的临床效果。方法:2017年1月-2020年5月收治新生儿ABO溶血症患儿198例,按是否使用静脉丙种球蛋白分为A组和B组;A组使用静脉丙种球蛋白,B组未使用;A组按使用丙球蛋白的时间点分为A1亚组及A_(2)亚组,A_(1)组使用时间点在生后48 h内,A_(2)亚组使用时间在生后48 h。比较各组临床效果。结果:A_(1)组光疗时间及住院天数均明显短于B组,差异有统计学意义(P<0.05)。A_(2)组光疗时间明显短于B组,差异有统计学意义(P<0.05);A_(2)组与B组住院天数比较,差异无统计学意义(P>0.05)。各组出院时血色素、网织红细胞百分比及胆红素等指标水平比较,差异无统计学意义(P>0.05)。结论:静脉使用丙种球蛋白可以减少黄疸光疗时间,早期使用(生后48 h内)效果更明显,可以缩短住院时间。Objective:To analyze the clinical effect of intravenous administration of gamma globulin in the treatment of neonatal ABO hemolysis.Methods:From January 2017 to May 2020,198 neonates with ABO hemolytic disease were selected,they were randomly divided into the group A and the group B according to whether intravenous gamma globulin was used or not.The group A used intravenous gamma globulin.The group B did not use it.The group A was divided into the A_(1) subgroup and the A_(2) subgroup according to the time point of gamma globulin use.The group A_(1) use time point≤48 h after birth,the use time of the subgroup A_(2)>48 h after birth.We compared the treatment effects of the two groups.Results:The phototherapy time and the length of stay in the group A_(1) were significantly shorter than those in the group B,the difference was statistically significant(P<0.05).The time of phototherapy in the group A_(2) was significantly shorter than that in the group B,the difference was statistically significant(P<0.05).There was no statistically significant difference in the length of stay between the group A_(2) and the group B(P>0.05).There was no significant difference in the levels of hemoglobin,percentage of reticulocytes and bilirubin in each group at discharge from the hospital(P>0.05).Conclusion:Intravenous administration of gamma globulin can reduce the time of phototherapy for jaundice,early use(within 48 hours after birth)is more effective and can shorten the hospital stay.
