摘　　要：目的:观察动态心电图(DCG)对房颤(AF)伴长R-R间期的诊断临床价值。方法:2016年1月-2020年12月收治DCG显示AF伴长R-R间期患者83例,结合睡眠与长R-R间期的相关性分为睡眠无关组和睡眠相关组。比较两组长R-R间期次数、心室率、逸搏、晕厥、长R-R间期、逸搏心律发生率。结果:两组共发生12643次长R-R间期。睡眠相关组最快心室率、最慢心室率、平均心室率均高于睡眠无关组,差异有统计学意义(P<0.05);睡眠相关组逸搏发生率、晕厥发生率、长R-R间期发生率、逸搏心律发生率均低于睡眠无关组,差异有统计学意义(P<0.05)。结论:DCG对AF伴长R-R间期有较好检出效果,且AF伴长R-R间期更容易发生在夜间睡眠状态,若是非睡眠时有心室率低、长R-R间期、晕厥表现,需要加强重视,尽早治疗。Objective:To explore the clinical value of dynamic electrocardiogram(DCG)in atrial fibrillation with long R-R interval.Methods:From January 2016 to December 2020,83 patients with DCG showing AF with long R-R interval were treated,combined with the correlation between sleep and long R-R interval,they were divided into sleep unrelated group and sleep related group.We compared the frequency of long R-R interval,ventricular rate,escape,syncope,long R-R interval and escape rhythm between the two groups.Results:A total of 12643 long R-R intervals occurred in the two groups.In the sleep related group,the fastest ventricular rate,the slowest ventricular rate and the average ventricular rate were higher than those in the sleep unrelated group,the difference was statistically significant(P<0.05).In the sleep related group,the incidence of escape,syncope,long R-R interval and escape rhythm were lower than those in the sleep unrelated group,the difference was statistically significant(P<0.05).Conclusion:DCG has a good detection effect on AF with long R-R interval,AF with long R-R interval is more likely to occur in night sleep.If there are low ventricular rate,long R-R interval and syncope in non sleep,we need to pay more attention to it and treat it as soon as possible.
