针刺阳陵泉穴不同留针时间对慢性炎性高张力胆囊动力的影响

摘　　要：目的:观察针刺阳陵泉穴不同留针时间对慢性炎性高张力胆囊动力影响的时效规律,为临床治疗慢性胆囊炎确定留针时间提供参考。方法:20例慢性胆囊炎患者均予针刺阳陵泉穴治疗,不留针组针刺得气后行针15s立即起针,留针组针刺得气后行针15s,留针40min,每隔10min行针1次,每次15s。留针组采集针刺前、留针40min过程中、起针后40min内胆囊体积,不留针组只采集针刺前、起针后40min内胆囊体积。数据采集时间间隔均为5min。综合比较两组各时点胆囊收缩率。结果:不同留针时间下胆囊均产生规律的舒张-收缩运动,形成不同幅度的压力梯度,留针40min所形成的压力梯度较不留针大;多时点胆囊收缩率综合比较,留针组显著低于不留针组(P<0.05)。结论:针刺阳陵泉穴留针40min能够明显缓解胆囊的高张力状态,对胆囊动力的修复作用优于不留针。Objective:Observe the time-effect rule of the influence of acupuncture at Yanglingquan(GB 34)with different needle retention time on chronic inflammatory hypertonic gallbladder motility,and provide a reference for clinical treatment of chronic cholecystitis to determine the needle retention time.Methods:Twenty patients with chronic cholecystitis were all treated with acupuncture at Yanglingquan(GB 34).The needles in the non-needle group were treated with acupuncture for 15 s after de qi,the needle retention group,acupuncture was manipulated for 15 s after de qi,and the needle was retained for 40 min,and the needle was manipulated once every 10 min,each time for 15 s.In the needle retention group,the gallbladder volume was collected before acupuncture,during the 40 min retention period,and 40 min after the needle was lifted,and the no needle group only collected the gallbladder volume before acupuncture and 40 min after the needle was lifted.The data collection time interval is 5 min.The gallbladder contraction rate at each time point of the two groups was comprehensively compared.Results:The gallbladder produces regular diastolic-contraction motions under different needle retention times,forming pressure gradients of different amplitudes.The pressure gradient formed by needle retention for 40 minutes is larger than that of no needle retention.Conclusion:Acupuncture at Yanglingquan(GB 34)for 40 min can significantly relieve the high tension state of the gallbladder,and the effect of repairing gallbladder motility is better than that of no needle.
