摘  要:目的探讨振幅整合脑电图(amplitude integrated electroencephalogram,aEEG)在早期评价心肺脑复苏(car⁃dio pulmonary cerebral resuscitation,CPCR)患者脑功能预后中的价值。方法选取2017年2月-2019年3月在航天中心医院治疗的CPCR患者65例,检测发病72 h内aEEG,比较脑功能预后良好和不良患者差异。结果65例患者中,脑功能预后不良患者47例,脑功能预后良好患者18例;脑功能预后不良患者aEEG分级明显高于预后良好患者(P<0.05),其达到Ⅱ级和Ⅲ级比例分别为44.68%和48.94%;aEEG分级≥Ⅱ级预测脑功能预后不良的灵敏性和阴性预测值分别为93.62%和82.35%,明显高于aEEG分级Ⅲ级(P<0.05);aEEG分级≥Ⅱ级和Ⅲ级预测脑功能预后不良的特异性和阳性预测值比较差异无统计学意义(P>0.05)。结论aEEG在早期评价CP⁃CR患者脑功能预后中有较好的价值,值得临床使用。Objective To explore the value of amplitude integrated electroencephalogram(aEEG)in early evaluation of the brain function prognosis in the patients undergoing cardiopulmonary cerebral resuscitation(CPCR).Methods Natus Nicolet Monitor was used on 65 patients undergoing CPCR with different underlying diseases within 72 hours after onset.Glasgow-Pittsburgh scoring system for brain function was used to evaluate the brain function.The sensitivity,specificity,and positive and negative predicting values of good and poor prognosis by aEEG grade were analyzed.The differences between the patients with good prognosis of brain function(n=18)and those with poor prognosis(n=47)were analyzed.Results The proportions of higher aEEG grades in the patients with poor prognosis were higher than in the patients with good prognosis:44.68%and 48.94%of the patients with poor prognosis reached aEEG gradesⅡandⅢrespectively.The sensitivity and negative predictive value of aEEG grade≥Ⅱin predicting poor prognosis of brain function were 93.62%and 82.35%respectively,significantly higher than those of aEEG gradeⅢ(both P<0.05),and there were no significant differences in the specificity and positive predictive value of aEEG≥ⅡandⅢin predicting poor prognosis of brain function(both P>0.05).Conclusion aEEG has a good value in the early evaluation of the prognosis of brain function in patients with CPCR,and it is worthy of clinical use.
