单心动周期实时三维超声心动图在扩张型心肌病心力衰竭患者左室形态及功能评价中的应用价值

摘　　要：目的探讨单心动周期实时三维超声心动图(sRT-3DE)在扩张型心肌病(DCM)心力衰竭患者左室形态及功能评价中的应用价值。方法选取160例DCM心力衰竭患者作为观察组,另选取同期进行健康体检的160例普通健康人作为对照组,均行sRT-3DE检查,比较两组的左室常规容积参数、左室几何形态学参数、左室收缩期节段容积变化同步性参数等。结果观察组患者的舒张末期容积(LVEDV)、收缩末期容积(LVESV)均明显高于对照组,左室射血分数(LVEF)明显低于对照组(t分别=18.97、33.99、20.93,P均<0.05),舒张末期球形指数(EDSI)、收缩末期球形指数(ESSI)值均明显高于对照组(t分别=15.96、16.07,P均<0.05)。观察组的收缩期失同步指数(SDI)、舒张期失同步指数(DDI)、收缩末离散度(DISPES)、舒张末离散度(DISPED)、收缩前时间容积、收缩后时间容积水平均明显高于对照组(t分别=23.34、10.85、25.08、13.17、12.07、11.06,P均<0.05)。观察组患者LVEF水平与SDI呈明显负相关(r=-0.89,P<0.05)。结论sRT-3DE能够较为准确地评估DCM心力衰竭患者的左室形态、左室收缩功能及左室同步性,并且操作简单、用时短、可重复性好。Objective To investigate the value of single cycle real-time three-dimensional echocardiography(sRT-3DE)in evaluating left ventricular morphology and function in patients with dilated cardiomyopathy(DCM)and heart failure.Methods One hundred and sixty cases of DCM patients with heart failure were selected as the observation group,and one hundred and sixty healthy people were selected as the control group.All subjects were examined by sRT-3DE.The parameters of left ventricular volume,left ventricular geometry,left ventricular systolic segmental volume and synchrony were compared between the two groups.Results The left ventricular end-diastolic volume(LVEDV)and left ventricular end-systolic volume(LVESV)of the observation group were significantly higher than those of control group,and the left ventricular ejection fraction(LVEF)of the observation group was significantly lower(t=18.97,33.99,20.93,P<0.05).The end diastolic spherical index(EDSI)and end systolic spherical index(ESSI)of the observation group were significantly higher than those of control group(t=15.96,16.07,P<0.05).The systolic dyssynchrony index(SDI),diastolic dyssynchrony index(DDI),dispersion end systolic(DISPES),dispersion end diastolic(DISPED),pre-systolic time volume and post systolic time volume of the observation group were significantly higher than those of the control group(t=23.34,10.85,25.08,13.17,12.07,11.06,P<0.05).The LVEF level was negatively correlated with SDI in the observation group(r=-0.89,P<0.05).Conclusion sRT-3DE can accurately evaluate left ventricular morphology,left ventricular systolic function and left ventricular synchrony in patients with DCM heart failure.
