海岛基层医院行腹腔镜脾切除术治疗外伤性脾损伤的应用研究

摘　　要：目的探讨腹腔镜脾切除术(LS)在海岛基层医院外伤性脾损伤治疗中的应用。方法回顾性分析43例外伤性脾损伤行LS治疗患者的临床资料,总结分析患者的手术时间、术中出血量、术后引流量及时间、并发症及术后住院时间等指标。结果41例患者顺利完成LS,2例中转开腹手术。患者手术时间51~223 min,术中出血量40~580 ml,拔除引流管时间术后3~8 d,术后住院时间5~15 d。术后并发症6例,对症处理后均好转;未出现死亡、重度胰漏、感染等严重并发症。结论LS具有创伤小、出血少、手术时间短、并发症少、恢复快等优点,需掌握严格的适应证及禁忌证,在海岛基层医院用于外伤性脾损伤的治疗是安全可行的。Objective To explore the application of laparoscopic splenectomy in the treatment of traumatic splenic injury in island primiral hospital.Methods The clinical data of 43 patients with traumatic splenic injury treated with laparoscopic splenectomy were analyzed retrospectively.The operation time,intraoperative bleeding,postoperative drainage volume and time,complications and postoperative hospital stay were summarized and analyzed.Results Forty-one patients were completed successfully with laparoscopic splenectomy and 2 cases were converted to open surgery.The operation time was 51-223 min,intraoperative bleeding was 40-580 ml,the time of removing drainage tube was 3-8 days after operation,the postoperative hospital stay was 5-15 days.Postoperative complications occurred in 6 cases,which improved after symptomatic treatment.There were no serious complications such as death,severe pancreatic leakage and infection.Conclusion Laparoscopic splenectomy has the advantages of less trauma,less bleeding,short operation time,less complications and rapid recovery.Strict indications and contraindications should be mastered.It is safe and feasible to use it in the treatment of traumatic spleen injury in island primiral hospital.
