消瘿散结汤治疗甲状腺结节临床研究

摘　　要：目的:观察消瘿散结汤治疗甲状腺结节(TN)的临床疗效及对患者血清甲状腺过氧化物酶抗体(TPOAb)、甲状腺球蛋白抗体(TGAb)的影响。方法:将106例TN患者以随机数字表法分为对照组与治疗组各53例。对照组给予左旋甲状腺素片治疗,治疗组在对照组基础上给予消瘿散结汤治疗。比较2组治疗后临床疗效及治疗前后中医证候积分、甲状腺抗体、结节平均直径,并比较2组不良反应。结果:治疗组临床总有效率88.68%,高于对照组71.70%(P<0.05)。与同组治疗前比较,治疗后2组中医证候积分、血清TPOAb、TGAb水平均降低(P<0.05),结节平均直径减小(P<0.05);与对照组治疗后比较,治疗组治疗后中医证候积分、血清TPOAb、TGAb水平均较低(P<0.05),结节平均直径较小(P<0.05)。治疗组不良反应发生率5.66%,低于对照组22.64%(P<0.05)。结论:消瘿散结汤治疗TN疗效显著,可有效缓解临床症状,抑制结节进展,降低甲状腺自身抗体水平,且能在一定程度上减少西药治疗引起的不良反应。Objective:To observe the clinical effect of Xiaoying Sanjie tang on the treatment of thyroid nodules(TN)and its effect on serum thyroid peroxidase antibody(TPOAb)and thyroglobulin antibody(TGAb)in patients.Methods:A total of106 patients with TN were divided into the control group and the treatment group by random number table method,53 cases in each group.The control group was treated with levothyroxine tablets and the treatment group was additionally treated with Xiaoying Sanjie tang based on the treatment of the control group.The clinical effect after treatment,Chinese medicine syndrome scores,thyroid antibodies and average diameter of nodules before and after treatment as well as adverse reactions were compared between the two groups.Results:The total effective rate was 88.68%in the treatment group,higher than that of 71.70%in the control group(P<0.05).Compared with the same group before treatment,Chinese medicine syndrome scores,levels of serum TPOAb and TGAb in both groups were decreased after treatment(P<0.05),and the average diameter of nodules were decreased as well(P<0.05);Compared with the control group after treatment,the Chinese medicine syndrome score,levels of serum TPOAb and TGAb in the treatment group were lower(P<0.05),and the average diameter of nodules was smaller(P<0.05).The incidence of adverse reactions in the treatment group was 5.66%,lower than that of 22.64%in the control group(P<0.05).Conclusion:Xiaoying Sanjie tang has a significant effect on treating TN,which can effectively relieve clinical symptoms,inhibit nodule progression,reduce the level of thyroid antibodies,and reduce the adverse reactions caused by western medicine treatment to a certain extent.
