摘　　要：目的探讨白细胞分化抗原137(CD137)、脑钠肽(BNP)及胰岛素样生长因子1(IGF-1)水平变化与非ST段抬高急性冠脉综合征(ACS)患者缺血评估(GRACE)评分的关系。方法本研究纳入2018年6月至2019年7月接受治疗的120例非ST段抬高的ACS患者,根据GRACE评分分为3组:低危组(37例,GRACE评分<126分)、中危组(42例,GRACE评分126~147分)、高危组(41例,GRACE评分>147分)。对比3组CD137、BNP及IGF-1水平,分析三者与GRACE评分的关系。随访12个月,比较不同CD137、BNP、IGF-1表达水平患者的预后生存率。结果3组CD137、BNP及IGF-1水平比较:低危组<中危组<高危组(P<0.05);CD137、BNP及IGF-1水平与GRACE评分呈正相关(r=0.658,P=0.011;r=0.662,P=0.027;r=0.552,P=0.018)。120例患者1年死亡率为19.17%(23/120);CD137高表达、BNP高表达、IGF-1高表达及GRACE评分>147分者死亡率更高(P<0.05)。结论CD137、BNP、IGF-1与GRACE评分呈正相关,加强检测三者水平有利于评估非ST段抬高的ACS患者预后。Objective To explore the relationship between the changes in the levels of cluster of differentiation 137(CD137),brain natriuretic peptide(BNP)and insulin-like growth factor-1(IGF-1)and Global Registry of Acute Coronary Events(GRACE)score of patients with non-STsegment-elevation acute coronary syndrome(ACS).Methods The 120 patients with non-ST-segment elevation ACS who were treated in our hospital from June 2018 to July 2019 were divided into 3 groups according to the GRACE score:37 cases in the low-risk group(GRACE score<126 points)and 42 cases in the intermediate-risk group(GRACE score 126-147),high-risk group(GRACE score>147).The CD137,BNP and IGF-1 levels among the three groups were compared,and the relationship between them and GRACE score was analyzed.The patients were followed up for 12 months.The prognostic survival rates of patients with different expression levels of CD137,BNP and IGF-1 were compared.Results The levels of CD137,BNP and IGF-1 in the three groups were as follows:low-risk group<intermediate-risk group<high-risk group(P<0.05).The levels of CD137,BNP and IGF-1 were positively correlated with GRACE score(r=0.658,P=0.011;r=0.662,P=0.027;r=0.552,P=0.018).The one-year mortality rate of 120 patients was 19.17%(23/120).Patients who had high expression of CD137,high expression of BNP,high expression of IGF-1,and GRACE score>147 had a higher mortality rate(P<0.05).Conclusion CD137,BNP,IGF-1 are positively correlated with GRACE scores.Strengthening the levels detection of the three is helpful to assess the prognosis of patients with non-ST-segment elevation ACS.
