改良宫颈冷刀锥切术治疗宫颈上皮内瘤变的临床研究

摘　　要：目的分析改良宫颈冷刀锥切术治疗宫颈上皮内瘤变(cervical intraepithelial neoplasia,CIN)的临床价值。方法回顾性分析2018年1月至2019年10月四川省妇幼保健院妇科收治的高级别CIN患者共86例,根据手术方法分为改良锥切组与常规锥切组,比较不同术式的临床疗效。结果改良锥切组患者的术中出血量(29.30±8.72)m L和手术时间(27.66±3.23)min相对于常规锥切组的术中出血量(47.49±7.79)m L、手术时间(36.28±2.05)min减少,差异有统计学意义(P <0.05)。术后改良锥切组锥体完整率(93.6%)高于常规锥切组(76.9%),锥尖偏离率(4.3%)低于常规锥切组(23.1%),差异均有统计学意义(P <0.05)。结论改良冷刀锥切术显著减少了手术时间和术中出血量,保证了切除标本的完整性及锥尖偏离的减少,值得临床推广。Objective To analyze the clinical value of modified cold-knife conization in the treatment of cervical intraepithelial neoplasia(CIN). Methods A retrospective analysis was conducted on 86 patients with the high-level CIN admitted to Sichuan Provincial Hospital for Women and Children from January 2018 to October 2019. According to the operation methods,they were divided into the modified cervical conization group and the conventional cervical conization group to compare the clinical efficacy. Results The intraoperative blood loss(29. 30 ± 8. 72)m L and operative time(27. 66 ± 3. 23)min of the patients in the modified cervical conization group were significantly less than those in the conventional cervical conization group [(47. 49 ± 7. 79)m L,(36. 28 ± 2. 05)min](P <0. 05). The rate of cone integrity(93. 6%)in the modified cervical conization group was higher than that in the conventional cervical conization group(76. 9%),the deviation rate of cone tip(4. 3%)was lower than that in the conventional cervical conization group(23. 1%),the differences were statistically significant(P < 0. 05). Conclusion The modified cervical conization can significantly reduce the operation time and intraoperative blood loss,and ensure the cone integrity and reduce the deviation of the cone tip,worthy of clinical promotion.
