超声指导下体外冲击波碎石治疗后尿道嵌顿结石的临床效果

摘　　要：目的分析超声指导下体外冲击波碎石治疗后尿道嵌顿结石的临床效果。方法根据随机数字表法将62例后尿道嵌顿结石患者分为对照组和观察组,每组31例。对照组给予膀胱镜大力钳碎石术治疗,观察组给予超声指导下体外冲击破碎石治疗。比较两组的临床效果。结果观察组的结石清除率高于对照组(P<0.05);两组术前及术后3、7 d的VAS评分依次降低(P<0.05);观察组术后3、7 d的VAS评分低于对照组(P<0.05)。观察组的并发症总发生率低于对照组(P<0.05)。术后1个月,观察组的Qmax高于对照组,RUV低于对照组(P<0.05)。术后24 h,观察组的钙、草酸、尿酸水平低于对照组(P<0.05)。结论超声指导下体外冲击波碎石治疗后尿道嵌顿结石可有效清除结石,改善患者尿生化指标水平,减轻疼痛,促进排尿功能恢复,安全可靠。Objective To analyze the clinical effect of ultrasound guided extracorporeal shock wave lithotripsy in the treatment of posterior urethral incarcerated stones.Methods Sixty-two patients with posterior urethral incarcerated stones were divided into control group and observation group by the random number table method,with 31 cases in each group.The control group was treated with cystoscopic forceps lithotripsy,and the observation group was treated with ultrasound guided extracorporeal shock wave lithotripsy.The clinical effects of the two groups were compared.Results The stone clearance rate in the observation group was higher than that in the control group(P<0.05);the VAS score in the two groups decreased gradually before operation and 3,7 d after operation(P<0.05);the VAS score in the observation group was lower than that in the control group at 3,7 d after operation(P<0.05).The total incidence of complications in the observation group was lower than that in the control group(P<0.05).One month after operation,the Qmax in the observation group was higher than that in the control group,and the RUV was lower than that in the control group(P<0.05).At 24 h after operation,the levels of calcium,oxalic acid and uric acid in the observation group were lower than those in the control group(P<0.05).Conclusion Ultrasound guided extracorporeal shock wave lithotripsy in the treatment of posterior urethral incarcerated stones can effectively remove stones,improve the level of urinary biochemical indexes,reduce pain and promote the recovery of micturition function of patients,with safety and reliability.
