摘 要:目的分析影响早产儿脐动脉血气(UABG)的相关因素。方法前瞻性收集榆林市第二医院2016年3月至2019年3月出生的早产儿194例,均检测UABG,包括pH、PaO_(2)、PaCO_(2)、BE、Lac、HCO_(3)^(-)。分析早产儿UABG各项指标与胎龄、脐带绕颈、妊高征、宫内窘迫、羊水污染程度、1 min Apgar评分的相关性。结果不同胎龄组早产儿PaO_(2)、HCO_(3)^(-)存在显著差异(P<0.05)。非妊高征组的PaO_(2)高于妊高征组(P<0.05)。羊水明显污染组的BE、HCO_(3)^(-)水平低于羊水无污染或羊水轻度污染组,Lac水平高于羊水无污染或羊水轻度污染组(P<0.05)。1 min Apgar评分≤7分组的pH、BE水平低于>7分组,PaCO_(2)、Lac水平高于>7分组(P<0.05)。结论妊高征、羊水严重污染和1 min Apgar评分≤7分均会增加早产儿窒息风险,UABG分析联合Apgar评分有助于准确判断早产儿窒息情况。Objective To analyze the related factors affecting umbilical artery blood gas(UABG)in premature infants.Methods A total of 194 premature infants born in Yulin No.2 hospital from March 2016 to March 2019 were prospectively collected.UABG was detected,including pH,PaO_(2),PaCO_(2),BE,Lac and HCO_(3)^(-).The correlation between UABG indexes of premature infants and gestational age,umbilical cord around neck,pregnancy induced hypertension,intrauterine distress,amniotic fluid pollution degree and 1 min Apgar score were analyzed.Results There were significant differences in PaO_(2) and HCO_(3)^(-)among the different gestational age groups(P<0.05).The PaO_(2) in the non-pregnancy induced hypertension group was higher than that in the pregnancy induced hypertension group(P<0.05).The BE and HCO_(3)^(-) levels in the amniotic fluid significantly polluted group were lower than those in the amniotic fluid non-polluted or slightly polluted group,and the Lac level was higher than that in the amniotic fluid non-polluted or slightly polluted group(P<0.05).The pH and BE level in the group with 1 min Apgar score≤7 were lower than those in the group with 1 min Apgar score>7,the PaCO_(2) and Lac level were higher than those in the group with 1 min Apgar score>7(P<0.05).Conclusion Pregnancy induced hypertension,severe amniotic fluid pollution and 1 min Apgar score≤7 increase the risk of asphyxia in premature infants.UABG analysis combined with Apgar score is helpful to accurately judge the asphyxia of premature infants.
