摘　　要：目的探讨CT引导下经皮肺穿刺活检术的临床应用价值。方法回顾性分析2015年1月~2019年12月在承德医学院附属医院行肺穿刺活检术的429例的临床资料,分析穿刺准确率、主要并发症的发生率及影响因素。结果429名患者均成功完成肺穿刺活检,病理结果恶性病变344例,良性病变84例,无明确诊断2例,准确率达98.3%;术后并发症主要为气胸及出血,并发气胸的有86例(20.0%),咯血77例(17.9%),心包积液1例(0.23%)。气胸并发症与进针深度有关,进针超过5cm以上气胸发生率较高。结论CT引导下肺穿刺活检术有助于肺部占位病灶的定性诊断,具有较高准确率,是肺部病变明确诊断的重要手段之一。Objective To discuss the clinical value of CT-guided percutaneous pulmonary biopsy.Methods We retrospectively analysed 429 patients who underwent CT-guided percutaneous pulmonary biopsy of Affiliated Hospital of Chengde Medical University from January 2015 to December 2019,and analyzed the puncture accuracy,incidence of major complications and influencing factors.Results All 429 patients completed lung biopsy successfully,344 patients were diagnosed as malignant lesions,84 patients were diagnosed as benign lesions,2 patients had no definite diagnosis,the accuracy rate is 98.3%.The main complications were pneumothorax and pulmonary hemorrhage,with pneumothorax in 86 cases(20.0%) hemoptysis in 77 cases(17.9%) and pericardial effusion in 1 case(0.23%).Pneumothorax complications were related to the depth of injection,and the incidence of pneumothorax was higher when the needle depth was more than 5 cm.Conclusion CT-guided percutaneous pulmonary biopsy is helpful for the diagnosis of lung space-occupying lesions with high accuracy,and it is one of the important methods for the definite diagnosis of lung lesions.
