摘 要:目的:探讨生姜泻心汤对感染难辨梭菌小鼠的治疗作用。方法:采用C57BL/6小鼠难辨梭菌感染模型,将小鼠分为正常组、模型组、万古霉素组(50 mg/kg)和万古霉素联合生姜泻心汤低剂量组(万古霉素50 mg/kg+生姜泻心汤16.4 g/kg),以及生姜泻心汤低剂量组(16.4 g/kg)、高剂量组(32.8 g/kg),所有小鼠连续给药7 d。通过体重、艰难梭菌毒素A&B水平和肠组织病理苏木精-伊红染色情况等评价生姜泻心汤治疗难辨梭菌感染的疗效。结果:造模前至造模成功时,各组小鼠体重稳步升高;造模成功后至治疗中期,小鼠体重持续降低,其中模型组降低最明显;治疗期,除生姜泻心汤高剂量组小鼠在治疗第5日的体重低于模型组,其他监测时间点,模型组小鼠体重显著低于万古霉素组、万古霉素联合生姜泻心汤低剂量组和生姜泻心汤低剂量组,差异均有统计学意义(P<0.05);观察期,各组小鼠体重仍持续升高。万古霉素组、万古霉素联合生姜泻心汤低剂量组小鼠的难辨梭菌毒素A&B水平得到有效降低,与模型组相比,差异有统计学意义(P<0.01/0.05);停药观察第5日,难辨梭菌毒素A&B水平较观察第2日升高,与模型组的差异均无统计学意义(P>0.05)。生姜泻心汤低剂量组小鼠治疗3 d后的艰难梭菌毒素A&B水平降至正常水平,与模型组相比,差异有统计学意义(P<0.01/0.05),且该水平维持至停药观察5 d。肠组织病理苏木精-伊红染色结果显示,治疗后,除生姜泻心汤高剂量组小鼠肠组织局部黏膜下层可见少量淋巴细胞浸润外,其他各组均未见肠组织有明显损伤。结论:生姜泻心汤单独使用的抑菌能力虽不及万古霉素,但仍表现出了一定的治疗作用;与万古霉素单独使用相比,生姜泻心汤低剂量联合万古霉素对毒素水平的抑制能力较稳定。生姜泻心汤的治疗作用稳定,起效较慢,且无复发迹象,体现了中药多成分、多靶点的作用特点�OBJECTIVE:To probe into the therapeutic effect of Shengjiang Xiexin decoction on mice infected with Clostridium difficile(C.difficile).METHODS:The C57BL/6 mice infected with C.difficile were divided into the normal group,model group,vancomycin group(50 mg/kg),vancomycin combined with Shengjiang Xiexin decoction low-dose group(vancomycin 50 mg/kg+Shengjiang Xiexin decoction 16.4 g/kg),and Shengjiang Xiexin decoction low-dose group(16.4 g/kg)and high-dose group(32.8 g/kg).All mice were dosed continuously for 7 d.The efficacy of Shengjiang Xiexin decoction in the treatment of C.difficile.infection was evaluated based on the mortality,body weight,A&B level of C.difficile,and HE staining of intestinal histology.RESULTS:The body weight of mice in each group increased steadily from before modeling to the time of successful modeling;after successful modeling to the middle of treatment,the body weight of mice continued to decrease,with the most obvious decrease in the model group.During the treatment,the body weight of mice in the model group was significantly lower than that in the vancomycin group,the vancomycin combined with Shengjiang Xiexin decoction low-dose group,and the Shengjiang Xiexin decoction low-dose group at other monitoring time points,with statistically significant difference(P<0.05),except for the body weight of mice in the Shengjiang Xiexin decoction high-dose group was lower than that in the model group on the 5th day of treatment.During the observation period,the body weight of mice in each group continued to increase.Compared with the model group,the A&B level of C.difficile decreased effectively in the vancomycin group and the vancomycin combined with Shengjiang Xiexin decoction low-dose treatment group(P<0.01/0.05).On the 5th day of drug withdrawal,the A&B level of C.difficile increased compared with the 2nd day of observation in the vancomycin treatment group and the vancomycin combined with Shengjiang Xiexin decoction low-dose treatment group,and there was no statistical significance compared wit
