静脉外膜戊二醛交联抑制人大隐静脉桥内膜增生的体外灌注研究

摘  要:目的:建立人大隐静脉桥(SVG)体外灌注模型,模拟动脉循环,探讨静脉外膜戊二醛交联抑制静脉桥内膜增生(IH)的效果。方法:冠状动脉旁路移植术中获取SVG均分两段,分为两组:交联组(静脉外膜行0.3%戊二醛交联5 min,n=30)和对照组(n=30)。两组SVG均连接体外灌注系统,在平均搏动压力120/80 mmHg(1 mmHg=0.133 kPa)下连续灌注7 d。Masson染色定量检测静脉桥IH;增殖细胞核抗原(PCNA)和血管细胞黏附分子1(VCAM-1)免疫组化染色半定量分析IH相关分子表达。结果:Masson染色显示交联组静脉内膜厚度显著低于对照组[(50.2±5.6)vs.(75.7±6.8)um,P<0.05];静脉外膜戊二醛交联可显著降低SVG内膜-中膜增厚比率达44.4%。免疫组化显示交联组SVG的PCNA(17.6%vs.26.4%)和VCAM-1(14.5%vs.48.6%)表达比例显著低于对照组(P<0.05)。结论:在短期模拟动脉循环的体外灌注环境下,静脉外膜戊二醛交联显著抑制人大隐静脉桥内膜增生。Objective:To research the effect of venous adventitia cross-link by glutaraldehyde reducing intimal hyperplasia(IH)of human saphenous vein graft(SVG)in an in vitro pulsatile perfusion model mimicking human arterial circulation.Methods:The SVG obtained from coronary artery bypass grafting was divided into two sections equally and then assigned two groups:cross-link group(SVG undergoing adventitial cross-link by 0.3%glutaraldehyde with 5 min,n=30)and control group(n=30).SVGs in two groups were connected to an in vitro pulsatile perfusion system for 7-day perfusion under an average pulsatile pressure of 120/80 mmHg.Masson staining quantitatively analyzed IH of SVG;immunohistochemical staining of proliferating cell nuclear antigen(PCNA)and vascular cell adhesion molecule 1(VCAM-1)semi-quantitatively analyzed the IH-related molecules expression.Results:Masson staining showed that the thickness of intima in cross-link group was significantly lower than that in control group[(50.2±5.6)vs.(75.7±6.8)um,P<0.05];venous adventitia cross-link by glutaraldehyde could significantly reduce the intima-media thickness ratio of SVG up to 44.4%.Immunohistochemistry staining showed that the expression ratio of PCNA(17.6%vs.26.4%)and VCAM-1(14.5%vs.48.6%)of SVG in the cross-linked group was significantly lower than that in the control group,P<0.05.Conclusions:Under mimicked arterial circulation and short-term in vitro perfusion environment,venous adventitia cross-link by glutaraldehyde significantly inhibited IH of SVG.
