不同入路方式微创经椎间孔腰椎椎体间融合术治疗单节段退变性腰椎滑脱的治疗效果对比

摘　　要：目的探讨不同入路方式微创经椎间孔腰椎椎体间融合术治疗单节段退变性腰椎滑脱的治疗效果。方法选取2017年3月-2020年4月收治的单节段退变性腰椎滑脱患者200例,按随机数字表法分为正中入路组(100例)和Wiltse入路组(100例)。正中入路组采用正中入路MIS-TLIF,Wiltse入路组采用Wiltse入路MIS-TLIF。对比两组手术基本情况、VAS评分和ODI评分。结果Wiltse入路组的手术时间、术中出血量、术后引流量、下地活动时间及住院时间均明显小于正中入路组(P<0.05);术后30d Wiltse入路组VAS评分显著低于正中入路组(P<0.05);术后6个月Wiltse入路组ODI评分显著低于正中入路组(P<0.05)。结论Wiltse入路MIS-TLIF应用于单节段退变性腰椎滑脱患者可有效改善患者手术基本情况,有利于降低患者术后疼痛和术后腰椎功能恢复,效果显著,值得临床推广。Objective To investigate the therapeutic effect of minimally invasive transforaminal lumbar interbody fusion with different approaches in the treatment of single-level degenerative lumbar spondylolisthesis. Methods A total of 200 patients with single-level degenerated lumbar spondylolisthesis admitted to our hospital from March 2017 to April 2020 were selected and divided into median approach group(100 cases) and Wiltse approach group(100 cases) according to random number table method. The median approach MIS-TLIF was used in the median approach group, and the Wiltse approach MIS-TLIF was used in the Wiltse approach group. The surgical information, VAS score and ODI score were compared between the two groups. Results The operative time, intraoperative blood loss, postoperative drainage volume, time of floor movement and hospital stay of Wiltse approach group were significantly lower than those of median approach group(P<0.05). 30 days after surgery, the VAS score of Wiltse approach group was significantly lower than that of median approach group(P<0.05). Six months after surgery, the ODI score of the Wiltse approach group was significantly lower than that of the median approach group(P<0.05). Conclusions The application of WILTSE approach MIS-TLIF in patients with single-level degenerative lumbar spondylolisthesis can effectively improve the basic surgical situation of patients, and is conducive to reducing postoperative pain and postoperative functional recovery of the lumbar spine. The effect is significant and worthy of clinical promotion.
