摘 要:目的通过网络数据库保存和管理新生儿耳廓筛查、复诊和矫正图片,并自动完成发送和接收短信对新生儿家长进行跟踪随访,探讨该系统在新生儿耳廓筛查中的应用价值。方法听力筛查医生在听力筛查同时进行耳廓筛查,发现异常登录自主研发手机APP"耳廓图库"拍照,录入母亲姓名或者手机号一键上传"图库管理和随访系统"。系统同时推送1周门诊复诊短信,复诊再次登录手机APP拍照上传对比恢复情况,建议矫正或1周后再复诊,同时推送第2次复诊短信,再次复诊操作同前。专人负责,收集患儿耳廓畸形恢复情况,复诊评价录入系统中。结果新生儿耳廓筛查图库管理和随访系统实现掌上移动平台管理功能,具有随时随地查询功能。实时掌握耳廓筛查、复诊和矫正图片的转归,具有统计报表功能,实现多方面数据检索功能,并可转换成EXCEL,方便统计软件处理分析。结论系统使用使临床医生第一时间能得到耳廓形态畸形自然转归统计数据,指导临床筛查医生对耳廓畸形判断是否需要矫正或者何时矫正具有指导意义。系统对家长进行短信提醒,对按时复诊、转诊起到重要作用,避免因为家长疏忽或者基层医院无该项技术而错过最佳矫正时期,很好贯彻了耳廓矫正"越早矫正,效果越好,佩戴时间越短"的理念,同时也避免事后医疗纠纷。系统对矫正患儿定期随访,使临床矫正医师及时了解矫正效果和经验总结具有重要意义。长期应用,对区域性耳廓畸形发病率、分类、自然转归是不可多得的大数据。Objective To study the value of a neonatal auricular screening system we developed that can store and manage images at neonatal auricular screening, follow-up visits and after correction in a web-based database, and automatically send and receive text messages from parents for follow-up purposes. Methods A physician examined the auricle at the time of newborn hearing screening and, if abnormality found, logged in a custom-developed mobile phone app( "Auricle Gallery") to take pictures and enter the mother’s name or phone number to be uploaded to the "Gallery Management and Follow-up System". The system would push out a follow-up reminder text message at 1-week, and a physician would log into the app again to take follow-up pictures to be uploaded for the purpose of comparison to determine recovery and for consultation on correction or continued observation. The process was repeated as needed. A dedicated staffer was responsible for monitoring auricular deformity recovery and follow-up data entry into the system. Results The app system allowed management of the neonatal auricle screening images database and follow-ups on a mobile platform and enabled information inquiry at any times and from any locations. The system’s other functions included graph and report generation and data retrieval, which could be exported as EXCEL files and processed using statistical software for analysis. Conclusion This is the first system that enables clinicians to monitor outcomes of newborn auricle deformity and guide the clinician in timing correction of the auricle deformity. SMS reminders for parents are important for timely follow-up visits and referrals, critical in avoiding loss of optimal correction time window due to parental negligence or lack of this technology in primary care units, and in minimizing the risk of related medicolegal disputes.
