ApoⅢ、APN及瘦素在妊娠糖尿病患者分娩前后的水平变化观察

摘　　要：目的探究血清载脂蛋白Ⅲ(ApoⅢ)、脂联素(APN)及瘦素在妊娠糖尿病患者分娩前后的变化。方法选取50例妊娠糖尿病患者为观察组,另选取50例同期待产的正常糖耐量孕妇为对照组,两组均在妊娠28周及分娩后6周检测ApoⅢ、APN及瘦素水平,比较两组产妇妊娠28周及分娩后ApoⅢ、APN、瘦素水平、不良事件发生情况;比较两组新生儿不良事件发生情况;分析观察组ApoⅢ、APN、瘦素水平相关性。结果妊娠28周,观察组ApoⅢ、瘦素均高于对照组,APN低于对照组,差异有统计学意义(P<0.05)。分娩后,两组ApoⅢ、瘦素均较妊娠28周降低,但观察组仍高于对照组;两组APN均较妊娠28周升高,但观察组仍低于对照组,差异均有统计学意义(P<0.05)。观察组不良事件的发生率为16.0%,高于对照组的4.0%,差异有统计学意义(P<0.05)。观察组新生儿不良事件事件发生率为18.0%,高于对照组的4.0%,差异有统计学意义(P<0.05)。Pearson相关性分析显示:ApoⅢ与APN呈负相关关系(P<0.05);ApoⅢ与瘦素呈正相关关系(P<0.05);APN与瘦素呈负相关关系(P<0.05)。结论妊娠糖尿病患者分娩后ApoⅢ、瘦素水平会出现下降,APN水平上升,与患者病情存在较大关联。Objective To investigate the changes of apolipoproteinⅢ(ApoⅢ),adiponectin(APN)and leptin in gestational diabetes mellitus patients before and after delivery.Methods 50 patients with gestational diabetes mellitus were selected as the observation group,and 50 pregnant women with normal glucose tolerance who were expecting delivery at the same time were selected as the control group.ApoⅢ,APN and leptin levels were measured at 28 weeks of gestation and 6 weeks after delivery in both groups,and ApoⅢ,APN,leptin levels and occurrence of adverse events were compared between the two groups at 28 weeks of gestation and after delivery;the incidence of neonatal adverse events was compared between the two groups;the correlation between ApoⅢ,APN and leptin levels in the observation group was analyzed.Results At 28 weeks of gestation,the ApoⅢand leptin in the observation group were higher than those in the control group,and APN was lower than that in the control group.All the differences were statistically significant(P<0.05).After delivery,the ApoⅢand leptin in the two groups were lower than those at 28 weeks of gestation,but the observation group was still higher than the control group;APN in both groups were higher than that at 28 weeks of gestation,but the observation group was still lower than the control group.All the differences were statistically significant(P<0.05).The incidence of adverse events in the observation group was 16.0%,which was higher than 4.0%in the control group,and the difference was statistically significant(P<0.05).The incidence of neonatal adverse events in the observation group was 18.0%,which was higher than 4.0%in the control group,and the difference was statistically significant(P<0.05).Pearson correlation analysis showed that ApoⅢwas negatively correlated with APN(P<0.05);ApoⅢwas positively correlated with leptin(P<0.05);and APN was negatively correlated with leptin(P<0.05).Conclusion ApoⅢand leptin levels decrease and APN levels increase after delivery in patients with gesta
