摘 要:目的:分析甲状腺癌通过高频彩超检查进行早期诊断的临床价值。方法:本研究共146例研究对象,均为2017年12月至2020年12月来我院检查的疑似甲状腺癌患者,患者均行高频彩超与二维超声检查。将病理学检查作为参照标准,比较检查结果。结果:在阳性预测值上,高频彩超检查97.56%略高于二维超声检查93.33%(P>0.05);在检出率、准确率、敏感度、特异度以及阴性预测值上,高频彩超检查95.21%、96.77%、96.77%、86.36%、82.61%明显高于二维超声检查77.40%、79.03%、79.03%、68.18%、36.59%(P<0.05)。在Ⅰ级与Ⅱ级血流占比上,高频彩超检查13.70%、31.51%低于二维超声检查29.45%、52.05%;在Ⅲ级血流占比上,高频彩超检查54.79%高于二维超声检查18.49%(P<0.05)。在阻力指数与动脉收缩期峰值速度上,高频彩超检查明显高于二维彩超检查(P<0.05)。结论:高频彩超用于甲状腺癌的早期诊断价值理想,其敏感度和特异度较高,临床可进一步推广运用。Objective:To analyze the clinical value of early diagnosis of thyroid cancer by highfrequency color Doppler ultrasound.Methods:A total of 146 subjects in this study were all suspected thyroid cancer patients who came to our hospital for examination from December 2018 to December2020.The patients underwent high-frequency color ultrasound and two-dimensional ultrasound examination.The pathological examination was used as a reference standard,and the examination results were compared.Results:In terms of positive predictive value,97.56%of high-frequency color Doppler ultrasound examination was slightly higher than 93.33%of two-dimensional ultrasound examination(P>0.05);in terms of detection rate,accuracy,sensitivity,specificity and negative predictive value,high frequency The 95.21%,96.77%,96.77%,86.36%and 82.61%of color Doppler ultrasound examinations were significantly higher than those of 77.40%,79.03%,79.03%,68.18%,36.59%of two-dimensional ultrasound examinations(P<0.05).In the proportion of gradeⅠand gradeⅡblood flow,13.70%and 31.51%of high-frequency color ultrasound examination are lower than 29.45%and 52.05%of two-dimensional ultrasound examination;in the proportion of gradeⅢblood flow,54.79%of high-frequency color ultrasound examination is higher than Two-dimensional ultrasound examination was18.49%(P<0.05).In terms of resistance index and peak arterial systolic velocity,high-frequency color Doppler ultrasound examination was significantly higher than that of two-dimensional color Doppler ultrasound examination(P<0.05).Conclusion:High-frequency color Doppler ultrasound examination has ideal value for early diagnosis of thyroid cancer,and its sensitivity and specificity are high,and it can be further promoted in clinical practice.
