摘  要:目的探讨多囊卵巢综合征(polycystic ovary syndrome,PCOS)患者血清性激素水平与胰岛素释放峰值后移的相关性。方法选取2019年11月~2021年1月在佛山市南海区妇幼保健院生殖健康科就诊PCOS患者120例,行胰岛素释放试验(insulin release test,IRT)和血清性激素检测。血清性激素检测包括雌二醇(estradiol,E2)、孕酮(progesterone,PG)、黄体生成素(luteinizing hormone,LH)、卵泡刺激素(follicle stimulating hormone,FSH)、泌乳素(prolactin,PRL)和睾酮(testosterone,T)。根据IRT结果进行分组,峰值时间在30或者60min,纳入无峰值后移组(n=60),峰值时间在120或者180 min,纳入峰值后移组(n=60)。比较两组血清性激素水平的差异,并分析性激素水平与胰岛素释放峰值后移的相关性。结果峰值后移组血清LH和FSH水平高于无后移组(t=2.369,2.066,P=0.019,0.041),血清E2峰值后移组低于无后移组(t=2.157,P=0.033),差异均有统计学意义;两组血清PG,PRL和T比较差异无统计学意义(Z=0.458,P=0.648;t=0.089,P=0.930;t=0.191,P=0.850)。Pearson相关性分析结果表明血清PG,PRL和T与胰岛素释放峰值后移无相关性(r=-0.042,0.008,0.018,均P>0.05),LH和FSH与胰岛素释放峰值后移呈显著正相关(r=0.213,0.187,均P<0.05),而E2与胰岛素释放峰值后移呈显著负相关(r=-0.195,P<0.05)。结论PCOS患者峰值后移与血清E2,LH,FSH存在相关性,可为阐明胰岛素释放峰值后移的机制提供参考依据。Objective To investigate the relationship between the serum sex hormone level and the insulin release peak shift in patients with polycystic ovary syndrome(PCOS).Methods Insulin release test(IRT)and serum sex hormones were performed in 120 patients with PCOS from November 2019 to January 2021 at the Reproductive Health Department of Nanhai District Maternal and Child Health Hospital in Foshan.The serum sex hormone included estradiol(E2),progesterone(PG),luteinizing hormone(LH)、follicle stimulating hormone(FSH),prolactin(PRL)and testosterone(T).The subjects were divided into two groups according to the IRT.Those with a peak time of 30 or 60 minutes were included in the non-peak-shift group(n=60),and those with a peak time of 120 or 180 minutes were included in the peak-shift group(n=60).The differences of serum sex hormone levels between the two groups were compared and the correlation between sex hormone levels and the backward shift of insulin peak was analyzed.Results The levels of serum LH and FSH in insulin peak shift group were higher than those in insulin peak non-shift group(t=2.369,2.066,P=0.019,0.041),the levels of serum E2 were lower than those insulin peak non-shift group(t=2.157,P=0.033),and the differences of them were statistically significant,respectively.However,the serum PG,PRL and T of the two groups were not statistically significant(Z=0.458,P=0.648;t=0.089,P=0.930;t=0.191,P=0.850,P>0.05).Pearson correlation analysis showed that there was no correlation between serum PRL,T,PG and insulin release peak shift(r=-0.042,0.008,0.018,all P>0.05),LH and FSH were positively correlated with insulin release peak shift(r=0.213,0.187,all P<0.05),while E2 was negatively correlated with insulin peak shift(r=-0.195,P<0.05).Conclusion Serum E2,LH and FSH were related with PCOS patients with insulin release peak shift,this may provide a reference for the determination of insulin peak shift.
