摘 要:目的通过分析飞行学员外周血压(SYS)及中心动脉压(CASP)的分布特点及其相关性,探讨中心动脉压的稳定性及其在飞行学员年度体检中的应用价值。方法选取航空大学及飞行学院共1760名飞行学员为研究对象,分别测量其中心动脉压、外周血压及心率(HR)、身高、体重、臂围等一般指标。根据外周血压值将其分为理想血压组(n=1084)、正常高值血压组(n=611)和异常血压组(n=65)。观察血压分布情况并分析其与脉率、身高、体重、臂围等一般指标相关性,同时比较理想血压组、正常高值血压组及异常血压组的中心动脉压与外周血压差异及相关性。结果1760名研究对象中,理想血压组1084例(61.6%),正常高值血压组611例(34.7%),异常血压组65例(3.7%);外周血压88~158 mmHg(1 mmHg=0.133kPa),平均(116.94±10.38)mmHg;中心动脉压范围73~138 mmHg,平均(100.8±9.21)mmHg;各组中心动脉压均小于外周血压,且差异有统计学意义,中心动脉压与外周收缩压差值(DV)分布在8~22 mmHg,平均(18.13±4.84)mmHg;通过皮尔逊相关性分析发现外周血压与脉率存在相关性(P<0.05),而与中心动脉压无显著相关性(P=0.793)。结论中心动脉压相比于外周动脉压稳定性更高,受身高、体重、臂围及脉率变化影响小,若能应用于飞行学员年度体检中,可以有效减少由于情绪波动等偶然因素导致的“假性高血压”的发生率。Objective To explore the application value of the stability of central arterial pressure(CASP)in flight physical examination by analyzing the distribution characteristics and correlation of peripheral blood pressure(SBP)and central arterial pressure(CASP)in flying cadets.Methods One thousand seven hundred sixty flight cadets from the aviation university and Flight College were selected as the research objects.The general indexes such as central arterial pressure,peripheral blood pressure,heart rate(HR),height,weight and arm circumference were measured.According to the peripheral blood pressure,they were divided into the ideal blood pressure group(n=1084),regular high blood pressure group(n=611)and abnormal blood pressure group(n=65).Observe the distribution of blood pressure and analyze its correlation with general indexes such as pulse rate,height,weight and arm circumference.At the same time,compare the difference and correlation between central arterial pressure and peripheral blood pressure in the ideal blood pressure group,regular high blood pressure group and abnormal blood pressure group.Results Among 1760 subjects,1084(61.6%)were in the ideal blood pressure group,611(34.7%)in the regular high blood pressure group and 65(3.7%)in the abnormal blood pressure group.The peripheral blood pressure was 88~158 mmHg(1 mmHg=0.133kPa),with an average of(116.94±10.38)mmHg.The central arterial pressure distribution range was 73~138 mmHg,with an average of(100.8±9.21)mmHg.The central arterial pressure in each group was lower than the peripheral blood pressure,which was statistically significant.The difference between central arterial pressure and peripheral systolic blood pressure(DV)was 8~22 mmHg,with an average of(18.13±4.84)mmHg;Pearson correlation analysis showed that there was a correlation between peripheral blood pressure and pulse rate(P<0.05),but there was no significant correlation with central artery pressure(P=0.793).Conclusion The stability of central arterial pressure is higher than that of peripheral a
