益气活血合去腐生肌法治疗老年褥疮的临床研究

摘　　要：目的探讨益气活血合去腐生肌法治疗老年褥疮的临床疗效指标。方法选取2019年5月至2021年4月来我院就医的80例褥疮患者,将患者随机平均分为两组,对照组按常规消毒方法,碘伏消毒疮面和周围皮肤清创,而后凡士林油纱、无菌敷料覆盖包扎。观察组在对照组基础上不用凡士林油纱,而是在褥疮处涂适量褥疮药膏,再用无菌敷料进行包扎,同时服用补阳还五汤。对治疗后7、15、30 d红斑面积及疗效等方面进行全面的观察与记录。结果治疗1个月后,对照组有效率为72.5%,明显低于观察组的95.0%,两组比较,差异有统计学意义(P=0.041<0.05),且观察组能显著缩小患者褥疮红斑面积。结论益气活血合去腐生肌法配合常规治疗方法治疗老年褥疮疗效显著,未见明显不良反应。Objective To investigate the clinical efficacy index in the treatment of senile bedsore with the method of benefiting qi for activating blood circulation combined with eliminating necrotic tissues and promoting granulation.Methods A total of 80 patients with bedsore admitted to our hospital from May 2019 to April 2021 were selected,and they were randomLy and equally divided into the control group(n=40)and the observation group(n=40).Patients in the control group were treated with conventional disinfection method.Iodophor was used to disinfect the wound surface and surrounding skin,and then debridement,vaseline gauze and sterile dressing were used to cover and bandage the wound.While on the basis of the control group,patients in the observation group were not treated with vaseline gauze.An appropriate amount of bedsore ointment mentioned in this paper was applied to bedsore.Then the patients were bandaged with sterile dressing and treated with Buyang Huanwu decoction at the same time.The erythema area and therapeutic efficacy were observed and recorded in overall aspects after 7,15 and 30 days of treatment.Results After 1 month of treatment,the statistical results showed that the effective rate of the control group was 72.5%,which was significantly lower than 95.0%of the observation group,with statistically significant difference between two groups(P=0.041<0.05).The observation group can significantly reduce the area of bedsore erythema in patients.Conclusion The method of benefiting qi for activating blood circulation combined with eliminating necrotic tissues and promoting granulation coordinated with conventional treatment is effective in treating senile bedsore,and no obvious adverse reactions are found.
