基于数据挖掘探析昆仑穴主治病症和配伍规律

摘　　要：目的:探析昆仑穴主治优势病症与腧穴配伍规律。方法:1949年10月前以《中华医典》和“全国报刊索引”为检索源,1949年10月后以中国知网、万方数据知识服务平台、维普期刊资源整合服务平台、PubMed等为检索源,根据纳入、排除标准筛选文献,建立SQL Server数据库,运用频次统计、关联规则Apriori算法及系统聚类算法,确定昆仑穴主治优势病症及配伍腧穴的频次、关联规则和聚类。结果:1949年10月前单穴及配伍优势病症分别为12种、14种,配伍腧穴170个,关联规则分析显示与委中、合谷、足三里、阳陵泉等穴联系紧密,聚类分析筛选出5类高频配伍腧穴组合;1949年10月后单穴及配伍优势病症分别为6种、9种,配伍腧穴156个,关联规则分析显示与环跳、阳陵泉、委中、肾俞等穴联系紧密,聚类分析选出6类高频配伍腧穴组合。结论:1949年10月前昆仑穴多用以治疗内科病症,且在耳鼻喉科的鼻衄和妇科的难产上显示出独特优势;1949年10月后多用以治疗肢体经络病症,对一些疼痛类病症疗效突出;配穴方面,多配伍五输穴、原穴、下合穴;配穴归经以膀胱经、胆经、大肠经为主。Objective:To explore the major indications and regularity of combination for Kunlun acupoint(BL60).Methods:The Chinese Medical Code(the Fifth Edition)and National Journal Index before October 1949 were searched and the literature after October 1949 was searched in China National Knowledge Internet(CNKI),Wanfang Database Knowledge Service Platform(WF),Weipu Journal Resource Integration Service Platform(VIP),PubMed and other databases.The retrieved data were screened based on the inclusion and exclusion criteria to establish a SQL Server database.Frequency statistics,association rule Apriori algorithm and systematic clustering algorithm were conducted to determine the frequency,association rules and clustering of the major indications and the combined acupoints of Kunlun.Results:In the literature before October 1949,12 disease are the major indications for single application,while 14 diseases are the major indications for combined application,totally with 170 combined acupoints;it is closely correlated with Weizhong(BL40),Hegu(LI4),Zusanli(ST36)and Yanglingquan(GB34)according to the association rule analysis,and there are 5 combinations with high frequency according to cluster analysis.In the literature after October 1949,6 diseases are the major indications for single application,while 9 diseases are the major indications for combined application,totally with 156 combined acupoints;it is closely correlated with Huantiao(GB30),Yanglingquan(GB34),Weizhong(BL40),Shenshu(BL23)according to the association rule analysis,and there are 6 combinations with high frequency according to cluster analysis.Conclusions:Before October 1949,the major indications for Kunlun are internal diseases,also with advantage in ophthalmic and otorhinolaryngologic diseases like epistaxis and gynecological disease like difficult delivery;after October 1949,the major indications of Kunlun are limb meridian diseases,with excellent effect on relieving pain;and on combination,five transport points,source points and lower sea point are the major one
