2015~2019年泰安市输入性卵形疟、三日疟实验室诊断及其病例分析

摘  要:目的了解泰安市输入性卵形疟及三日疟病例流行特征及实验室诊断情况,为制定疟疾防控策略提供科学依据。方法对泰安市2015~2019年输入性卵形疟及三日疟的市县级实验室镜检结果与省级疟疾诊断参比实验室镜检及巢氏PCR法的复核结果进行比对,并通过"中国疾病预防控制信息系统"和"寄生虫病防治信息管理系统",对相关病例监测和疫情报告资料进行流行病学分析。结果2015~2019年泰安市共报告输入性疟疾病例122例,其中卵形疟18例、三日疟病例7例,分别占14.75%和5.73%。两种病例占总报告数的20.49%,各年度间构成比差异无统计学意义(P>0.05)。三日疟实验室诊断符合率为85.71%(6/7),而卵形疟仅为66.67%(12/18),两种病例各年度间诊断符合率差异无统计学意义(P>0.05)。岱岳区和肥城市报告了全部7例输入性三日疟病例,而卵形疟病例主要集中在岱岳区、肥城市和泰山区3个区县。输入来源地主要集中于中非的赤道几内亚和西非的安哥拉等非洲国家。结论泰安市输入性卵形疟和三日疟病例在地区分布和输入来源上均有明显的地域分布特点。Objective To understand the epidemiological characteristics and laboratory diagnosis of imported Ovale malariaand Plasmodium malariaein Taian city during 2015-2019,so as to provide the evidence for improving the prevention and control of malaria.Methods The laboratory results of microscopy of imported Ovale malariaand Plasmodium malariaein Taian city during 2015-2019 in city-level and county-level was compared with the provincial laboratory results of microscopy and nest-PCR.And the final data were analyzed in epidemiology from the Information Management System for Infectious Diseases Report and Information Management System for Parasitic Diseases Control and Prevention.Results A total of 122 imported malaria cases were reported in Taian city during 2015-2019,including 18 cases of oval malaria and 7 cases of plasmodium malariae,accounting for 14.75%and 5.73%respectively.The 2 kinds of cases accounted for 20.49%of the total reported,and there was no statistically significant difference in the diagnostic coincidence rate between the 2 kinds of cases in each year(P>0.05).The coincidence rate of laboratory diagnosis of Plasmodium malariae was 85.71%(6/7),while that of oval malaria was only 66.67%(12/18),and there was no statistically significant difference in composition ratio between years(P>0.05).All 7 cases of imported Plasmodium malariae were reported in Daiyue district and Feicheng city,while the cases of ovale malaria were mainly in Daiyue district,Feicheng city and Taishan district.Imported sources were mainly concentrated in African countries such as Equatorial Guinea in central Africa and Angola in west Africa.Conclusion The imported cases of Ovale malariaand Plasmodium malariaein Taian city have obvious geographical distribution characteristics in terms of geographical distribution and input sources.
