摘 要:目的:探讨经皮射频靶点热凝联合臭氧消融在腰椎间盘突出症(LDH)患者髓核回缩效应及炎症反应中的效果。方法:选取2019年5月-2020年5月本院收治的84例LDH患者,按随机数字表法分为对照组42例和观察组42例。对照组使用经皮射频靶点热凝术,观察组加用臭氧消融术,随访至术后6个月。对比两组临床疗效、炎症因子水平、髓核回缩效应指标(髓核横径与前后径)。结果:观察组治疗总有效率为95.24%,高于对照组的78.57%,差异有统计学意义(P<0.05);术前,两组白介素-6(IL-6)、肿瘤坏死因子-α(TNF-α)水平、髓核横径与前后径相比,差异无统计学意义(P>0.05);观察组术后7 d的IL-6、TNF-α水平低于对照组,术后6个月的髓核横径与前后径小于对照组,差异有统计学意义(P<0.05)。结论:经皮射频靶点热凝联合臭氧消融可有效控制LDH患者机体炎症反应,提高髓核回缩效应,安全可行。Objective:To investigate the effect of percutaneous radiofrequency targeted thermocoagulation combined with ozone ablation on the retractive effect and inflammatory response of nucleus pulpoda in patients with lumbar disc herniation(LDH).Method:A total of 84 patients with LDH in our hospital from May 2019 to May 2020 were selected and randomly divided into the control group(42 cases)and the observation group(42 cases).The control group was treated with percutaneous radiofrequency target thermocoagulation,and the observation group was combined with ozone ablation.The patients were followed up to 6 months after surgery.The clinical efficacy,inflammatory factors,retraction effect of nucleus pulposus(transverse diameter and anterior posterior diameter of nucleus pulposus)were compared between the two groups.Result:The total effective rate of the observation group was 95.24%,which was higher than 78.57%of the control group(P<0.05).Before operation,there were no significant differences in the levels of IL-6 and TNF-α,transverse diameter,anterior and posterior diameter of nucleus pulposus(P>0.05).After 7 days of operation,the levels of IL-6 and TNF-αin the observation group were lower than those of the control group,and the transverse diameter,anterior and posterior diameter of nucleus pulposus after 6 months of operation in the observation group were smaller than those in the control group,and the differences were statistically significant(P<0.05).Conclusion:Percutaneous radiofrequency target thermocoagulation combined with ozone ablation can effectively control the inflammatory reaction of LDH patients and improve the retraction effect of nucleus pulposus,which is safe and feasible.
