益气温阳健脾方辅助治疗稳定期慢性阻塞性肺疾病肺脾肾气虚证的临床研究

摘　　要：目的:探讨益气温阳健脾方辅助治疗稳定期慢性阻塞性肺疾病(COPD)肺脾肾气虚证的临床效果。方法:选取2019年10月-2020年9月收治的104例稳定期COPD肺脾肾气虚证患者,按照随机数字表分为研究组和对照组,各52例。对照组给予沙美特罗替卡松吸入粉雾剂治疗,研究组在对照组基础上给予益气温阳健脾汤治疗,疗程3个月。比较两组中医症候积分、临床疗效、肺功能指标及安全性。结果:治疗前两组中医症候总积分比较,差异无统计学意义(P>0.05);治疗后两组中医症候总积分均低于治疗前,且研究组低于对照组(P<0.05);研究组临床总有效率高于对照组(92.31%vs 76.92%,P<0.05);治疗前两组第1秒用力呼气容积占预计值百分比(FEV_(1)%)、第1秒用力呼气容积与用力肺活量的百分比(FEV_(1)/FVC)比较差异无统计学意义(P>0.05);治疗后两组FEV_(1)%、FEV_(1)/FVC均高于治疗前,且研究组高于对照组(P<0.05);治疗期间两组均无不良反应出现。结论:益气温阳健脾方辅助治疗稳定期COPD肺脾肾气虚证患者的临床效果确切,可有效改善临床症状及呼吸功能,安全可靠。Objective:To explore the clinical effect of Yiqi Wenyang Jianpi Prescription in the adjuvant treatment of stable chronic obstructive pulmonary disease(COPD)lung spleen and kidney Qi deficiency syndrome.Method:A total of 104 patients with stable COPD lung spleen and kidney Qi deficiency syndrome who were admitted from October 2019 to September 2020 were selected and divided into the study group and the control group according to a random number table,with 52 cases in each group.The control group was treated with Salmeterol Xinafoate and Fluticasone Propionate Powder for Inhalation,and the study group was treated with Yiqi Wenyang Jianpi Prescription on the basis of the control group and treated for 3 months.The scores of TCM syndromes,clinical efficacy,pulmonary function indexes and safety were compared between the two groups.Result:There was no difference in the total score of TCM syndromes in the two groups before treatment(P>0.05).After treatment,the total scores of TCM syndromes in the two groups were lower than before treatment,and the study group was lower than the control group(P<0.05).The total clinical effective rate of the study group was higher than that of the control group(92.31%vs 76.92%,P<0.05).Before treatment,the percentage of forced expiratory volume in the first second to the predicted value(FEV_(1)%)and the percentage of forced expiratory volume in the first second of forced vital capacity(FEV_(1)/FVC)of the two groups had no significant differences(P>0.05).After treatment,FEV_(1)%and FEV_(1)/FVC in the two groups were higher than those before treatment,and which were higher in the study group than those in the control group(P<0.05).During the treatment,there was no adverse reaction in the two groups.Conclusion:Yiqi Wenyang Jianpi Prescription has a definite clinical effect in the adjuvant treatment of patients with stable COPD lung spleen kidney Qi deficiency syndrome,which can effectively improve clinical symptoms and respiratory function,and is safe and reliable.
