补肺化瘀通络汤联合阿替普酶治疗慢性肺栓塞疗效研究

摘　　要：目的:分析补肺化瘀通络汤联合阿替普酶治疗慢性肺栓塞疗效及对患者血浆D-二聚体(D-D)、肌钙蛋白(cTnI)、血清高迁移率族蛋白B1(HMGB-1)的影响。方法:将90例慢性肺栓塞患者以随机数字表法分为两组,每组45例。对照组予阿替普酶治疗,治疗组予补肺化瘀通络汤联合阿替普酶治疗。对比两组疗效及肺功能、血气指标、凝血功能及D-D、cTnI、HMGB-1水平。结果:治疗组总有效率为97.78%,高于对照组的77.78%(P<0.05)。治疗后治疗组FIB水平高于对照组,PT、TT、APTT水平低于对照组,差异有统计学意义(均P<0.05)。治疗后治疗组FEV1/FVC、FEV1高于对照组,治疗组D-D、cTnI、HMGB-1水平低于对照组,差异有统计学意义(均P<0.05)。治疗后治疗组PaCO2低于对照组,PaO2高于对照组,差异有统计学意义(均P<0.05)。结论:补肺化瘀通络汤联合阿替普酶治疗慢性肺栓塞疗效较好,可提高患者肺功能,改善凝血功能、血气指标,降低D-D、cTnI、HMGB-1水平。Objective:To analyze efficacy of Bufei Huayu Tongluo decoction combined with alteplase in the treatment of chronic pulmonary embolism and its effects on plasma D-dimer(D-D),cardiac troponin I(cTnI)and serum high mobility group protein B1(HMGB-1).Methods:90 patients were divided into two groups according to random number table method,45 cases in each group.The control group received alteplase treatment,and the treatment group received Bufei Huayu Tongluo decoction combined with alteplase treatment.Efficacy and lung function,blood gas index,coagulation function and D-D,cTnI,HMGB-1 were compared between the two groups.Results:The total effective rate of the treatment group was 97.78%higher than 77.78%of the control group(P<0.05).After treatment,the level of FIB in the treatment group was higher than that in the control group,while the levels of PT,TT,APTT were lower than those in the control group,difference statistically significant(all P<0.05).After treatment,the levels of FEV1/FVC,FEV1 in the treatment group were higher than those in the control group,and the levels of D-D,cTnI,HMGB-1 in the treatment group were lower than those in the control group,difference statistically significant(all P<0.05).After treatment,the level of PaCO 2 in the treatment group was lower than that in the control group,while the level of PaO 2 was higher than that in the control group,difference statistically significant(all P<0.05).Conclusion:Bufei Huayu Tongluo decoction combined with alteplase has good effect in the treatment of chronic pulmonary embolism,which can improve lung function,coagulation function,blood gas index,and reduce D-D,cTnI,HMGB-1 levels.
