电热针灸拔罐联合中频脉冲治疗腰椎间盘突出症伴坐骨神经痛疗效研究

摘　　要：目的:观察电热针灸拔罐联合中频脉冲治疗腰椎间盘突出(LDH)伴坐骨神经痛的临床疗效。方法:选取158例LDH伴坐骨神经痛患者为观察对象,按就诊顺序编号后通过抽签法随机分为联合组和对照组各79例,对照组患者给予中频脉冲治疗,联合组在对照组基础上联合电热针灸拔罐治疗,比较两组患者的临床疗效、症状综合积分、疼痛程度、运动功能、炎症因子水平。结果:联合组总有效率92.41%高于对照组的78.48%,差异有统计学意义(P<0.05)。治疗后,联合组症状综合积分、VAS评分、PPI评分、PRI评分、McGill总分、IL-6和TNF-α水平均低于治疗前和对照组,差异有统计学意义(均P<0.05),联合组腰椎关节前屈度、后伸度、Fugl-Meyer得分和β-EP水平均高于治疗前和对照组,差异有统计学意义(均P<0.05)。结论:电热针灸拔罐联合中频脉冲治疗LDH伴坐骨神经痛临床疗效较好,可有效改善患者综合症状、疼痛程度和运动功能,抑制炎症反应。Objective:To explore clinical efficacy of electrothermal acupuncture cupping combined with intermediate frequency pulse in the treatment of lumbar disc herniation(LDH)with sciatica.Methods:158 patients with LDH combined with sciatica were selected as observation subjects.The patients were numbered according to the order of treatment and then randomly divided into combined group and control group by lottery,with 79 cases in each group.The control group were given intermediate frequency pulse therapy,and the combined group were combined with electrothermal acupuncture cupping on the basis of the control group.The clinical efficacy,comprehensive symptoms scores,pain degree,motor function and levels of inflammatory factors were compared between the two groups.Results:The total effective rate of the combined group was 92.41%significantly higher than 78.48%of the control group(P<0.05).The scores of comprehensive symptoms,VAS score,PPI score,PRI score,McGill total score and levels of IL-6 and TNF-αof the combined group were significantly lower than those before treatment and the control group,difference statistically significant(all P<0.05).The anteflexion and rear protraction of the lumbar spine,Fugl-Meyer score and level ofβ-EP of the combined group were significantly high than those before treatment and the control group(all P<0.05).Conclusion:Electrothermal acupuncture cupping combined with intermediate frequency pulse has significant clinical efficacy in the treatment of LDH with sciatica.It can effectively improve comprehensive syndromes,pain degree and motor function,inhibit the inflammatory response.
