外化叙事心理干预对整形美容患者自我评估、自我效能及期望偏差的影响  

Effect of externalized narrative psychological intervention on self⁃evaluation,self⁃efficacy and expectation deviation of patients with plastic surgery

摘　　要：目的探究外化叙事心理干预对整形美容患者自我评估、自我效能及期望偏差的影响。方法随机选取于空军军医大学第一附属医院行整形美容手术的98例患者作为研究对象,按照随机数字表法将其分为观察组(n=50)和对照组(n=48),因研究总例数相对较少,故使用随机数字表法分组时2组例数不相同。其中,对照组给予常规护理,观察组在对照组的基础上实施外化叙事心理干预护理,比较2组患者的自评满意率、负性情绪、自我效能、应对方式、整形满意度及期望偏差。结果干预后,观察组自评满意率为94.00%,对照组为79.17%,2组对比差异有统计学意义(P<0.05);2组患者的体象障碍自评量表(BIRS)、凯斯勒心理困扰量表(K10)评分均显著降低(P<0.05),自我效能感量表(GSES)评分均显著升高(P<0.05),简易应对方式问卷(SCSQ)中积极应对评分均显著升高(P<0.05),消极应对评分均显著降低(P<0.05),且干预后上述指标组间对比差异有统计学意义(P<0.05);观察组整形满意率为92.00%,对照组为77.08%,2组对比差异有统计学意义(P<0.05);观察组术后期望偏差发生率为2.00%,对照组为12.50%,2组比较差异有统计学意义(χ^(2)=4.07,P<0.05)。结论外化叙事心理干预可显著增强整形美容患者的自信心,改善患者的不良情绪,提高患者的自我效能感及积极应对方式,且患者的期望偏差发生率较低。Objective To observe the effect of externalized narrative psychological intervention on self⁃evaluation,self⁃effica⁃cy and expectation deviation of patients undergoing plastic surgery.Methods Totally 98 patients who underwent plastic surgery treat⁃ment in First Affiliated Hospital of Air Force Medical University,were randomly selected as the research subjects.According to the ran⁃dom number table method,the patients were divided into observation group and control group.Control group(n=48)was given routine nursing,and observation group(n=50)was added with externalized narrative psychological intervention on the basis of control group.Becanse the total number of cases was relatively small,the number of two cases was different when grouped using random number ta⁃bles.Self⁃evaluation satisfaction rate,negative emotions,self⁃efficacy,coping styles,plastic satisfaction and expectation deviation were compared between the two groups.Results After intervention,the Self⁃evaluation satisfaction rate was 94.00% in observation group and was 79.17%in control group with statistical significance(P<0.05).The scores of Body Image Rating Scale(BIRS)and Kessler Psychological Distress Scale(K10)were significantly reduced(P<0.05)while the score of General Self⁃Efficacy Scale(GSES)was significantly increased(P<0.05),and the score of active coping of Simplified Coping Style Questionnaire(SCSQ)was significantly increased(P<0.05)while the score of negative coping was significantly decreased(P<0.05)and the comparison of the above indi⁃cators between the groups after intervention was statistical significance(P<0.05).The Plastic satisfaction rate was 92.00%in obser⁃vation group and was 77.08%in control group with statistical significance(P<0.05).The incidence rate of postoperative expectation deviation was 2.00%in observation group and 12.50% in control group(χ^(2)=4.07,P<0.05).Conclusion Externalized narrative psychological intervention for patients with plastic surgery can significantly enhance the self⁃confidenc
