壮肝益肾煎治疗乙肝合并酒精性肝硬化的临床观察

摘　　要：为了观察壮肝益肾煎治疗乙肝合并酒精性肝硬化的临床疗效,将120例诊断为乙肝合并酒精性肝硬化患者随机分为观察组和对照组各60例,对照组选取单纯西医治疗,治疗组在对照组的基础上加用壮肝益肾煎治疗,比较2组患者治疗前后肝功能、凝血功能、肿瘤标志物、免疫球蛋白、基质金属蛋白酶-2、基质金属蛋白酶-9、血管内皮生长因子、细胞增殖调控因子(转化生长因子-β1、胰岛素样生长因子-2)、炎症因子(白介素-2、肿瘤坏死因子-α、肝硬化指标)的变化。结果表明:治疗组患者在临床症状,肝硬化改变,肝功能、肝硬化指标、凝血功能、免疫球蛋白、自然杀伤细胞、CD4+T淋巴细胞、肿瘤标志物的改善情况上,血清中MMP-2、MMP-9、VEGF的分子表达水平明显优于对照组(P<0.05)。壮肝益肾煎治疗能改善患者的生化学指标、凝血功能,调节机体免疫状态,提高临床疗效。This study aims to observe the clinical effect of Zhuanggan Yishen decoction on hepatitis B complicated with alcoholic cirrhosis.A total of 120 patients diagnosed with alcoholic cirrhosis were randomly divided into treatment group and control group,with 60 cases in each group.The control group received with Western treatment while the treatment group received extra strong liver kidney decoction treatment.We compared the two groups before and after treatment in terms of liver function,blood coagulation function,tumor markers,immunoglobulin,matrix metalloproteinases 2,matrix metalloproteinases 9,vascular endothelial growth factor,regulation and control of cell proliferation,factors such as transforming growth factor beta 1,insulin-like growth factor 2,inflammation factors such aschanges of interleukin-2,tumor necrosis factor-αand cirrhosis indexes.The molecular expression levels of MMP-2,MMP-9 and VEGF in the treatment group were significantly better than those in the control group in terms of clinical symptoms,changes in liver cirrhosis,liver function,liver cirrhosis indexes,coagulation function,immunoglobulin,natural killer cells,CD4+T lymphocytes and AFP(P<0.05).The combination therapy of strengthening liver and nourishing kidney decoction can improve the biochemical indexes and blood coagulation function of patients,regulate the immune state of the body,improve the clinical efficacy and significantly alleviate the clinical symptoms,which proves that the combination therapy of traditional Chinese medicine in the treatment of alcoholic cirrhosis is more accurate.
