摘 要:目的探讨双源CT血管成像(CTA)评价主动脉瓣钙化(AVC)及狭窄应用价值。方法选取我院100例AVC患者纳入观察组,并选取同期50例非AVC受试者纳入对照组。使用CTA检测两组受试者主动脉瓣开放角、开口面积、瓣膜厚度,并分析主动脉瓣开口面积、钙化积分与主动脉瓣狭窄的相关性。结果观察组100例患者中,CTA共检出钙化主动脉瓣135枚,患者主动脉瓣钙化以后瓣常见,且多集中于瓣膜根部。观察组左瓣、右瓣、后瓣开放角及主动脉瓣开口面积均低于对照组,其左瓣、右瓣、后瓣瓣膜厚度均高于对照组,差异有统计学意义(P<0.05)。Pearson相关性分析显示不同钙化位置开放角与钙化积分均呈正相关,瓣膜根部钙化者开放角与钙化积分的相关性最为显著(P<0.05)。Spearman相关性分析显示钙化积分与主动脉瓣狭窄程度呈正相关(P<0.05)。结论主动脉瓣钙化好发于后瓣及瓣膜根部,且钙化位置、钙化积分与主动脉瓣开放角、主动脉狭窄程度存在密切关联,故对于AVC患者而言建议定期监测主动脉瓣钙化程度变化,以指导临床干预。Objective To evaluate the value of dual-source CT angiography(CTA)in evaluating the effects of aortic valve calcification(AVC)on its function.Methods A total of 100 AVC patients admitted to our Hospital from March 2018 to March 2021 were included in the study group,and 50 non-AVC subjects were included in the control group.CTA was used to detect the aortic valve opening angle,opening area,and thickness of the two groups of subjects,and the correlation between the aortic valve opening area,calcification score and aortic stenosis was analyzed.Results A total of 135 calcified aortic valves were detected by CTA among the 100 patients in the study group.The left,right and posterior valve opening angle and aortic valve opening area of the study group were lower than those of the control group,and the thickness of the left,right and posterior valve was higher than that of the control group,with statistical significance(P<0.05).Pearson correlation analysis showed that the opening angle of different calcification locations was positively correlated with the calcification score,and the valvular root calcification had the most significant correlation with the calcification score(P<0.05).Spearman correlation analysis showed that calcification score was positively correlated with the degree of aortic stenosis(P<0.05).Conclusion Aortic valve calcification usually occurs in the posterior valve and valve root,and the calcification location and calcification score are closely related to the aortic valve opening angle and the degree of aortic stenosis.Therefore,regular monitoring of the degree of aortic valve calcification is recommended for patients with AVC to guide clinical intervention.
