MSCTA诊断外伤性上肢动脉损伤的价值分析

摘　　要：目的探讨128层螺旋CT血管成像在外伤性上肢动脉损伤诊断中的应用价值。方法选取44例因外伤所致上肢动脉血管损伤患者的上肢动脉CTA影像资料,利用多种后处理技术观察血管损伤情况,并与数字减影血管造影术(DSA)或手术进行比较。结果44例中MSCTA阴性7例,阳性37例,损伤血管48条。损伤类型包括动脉部分破裂6条,动脉狭窄11条,动脉闭塞22条,动静脉瘘2条,假性动脉瘤3条,动脉断裂4条。MSCTA诊断上肢动脉损伤的结果与DSA或手术探查结果对比,诊断敏感性、特异性、符合率分别为97.87%、75.00%、94.55%。结论MSCTA可直观准确地显示上肢动脉损伤情况,对外伤所致上肢动脉损伤的患者具有较高的诊断价值和临床应用价值。Objective To study the application value of 128-slice spiral CT angiography in the diagnosis of traumatic upper extremity artery injury.Methods We retrospectively analysed the CTA imaging of 44 cases with upper extremity arterial injury caused by trauma.Vascular injury was observed by various CT reconstruction techniques and compared with DSA or surgery.Results Among the 44 patients,7 cases were negative and 37 cases were positive,including 48 arterial injuries.The types of injury included 6-partial arterial rupture,11-arterial stenosis,22-arterial occlusion,2-arteriovenous fistula,3-pseudoaneurysm and 4-arterial rupture.The sensitivity,specificity and coincidence rate of MSCTA in the diagnosis of upper extremity artery injury were 97.87%,75.00%and 94.55%,respectively.Conclusion MSCTA can directly and accurately display the upper extremity artery injury,which has high diagnostic value and clinical application value for patients with upper extremity artery injury caused by trauma.
