MR弹性成像对正常胰腺评价及胰腺占位性病变鉴别的临床价值

摘　　要：目的探讨正常胰腺和胰腺占位性病变,在磁共振弹性成像(MRE)检测中的平均弹性值差异鉴别胰腺占位性病变的诊断有效性。方法选取我院收治的胰腺肿瘤患者68例,其中胰腺导管腺癌40例,良性占位28例。另纳入同期体检健康志愿者20例为对照组。全部观察对象接受3.0T MR弹性成像检查,共振频率设定为40 Hz。结果胰腺癌组织的平均弹性值为4.33±1.69,胰腺良性占位平均弹性值为1.93±0.86,正常胰腺组织的平均弹性值为1.13±1.10,组间差异具有统计学意义(P<0.05);以弹性值≥1.325 kPa为诊断良性胰腺占位的ROC曲线下面积为0.748,敏感性为71.4%,特异性为100.0%;以弹性值≥3.310 kPa为鉴别良、恶性胰腺占位的ROC曲线下面积为0.871,敏感性为75.0%,特异性为89.3%。结论MRE可以通过测定组织弹性,有效区分正常胰腺及胰腺占位性病变的良、恶性,对鉴别诊断具有较高的临床应用价值。Objective To study the difference in mean elasticity between healthy pancreas and pancreatic space occupying lesions in magnetic resonance elastography(MRE),and the effectiveness of this technique in the diagnosis of pancreatic space occupying lesions.Methods The study included 68 patients with pancreatic cancer from January 2019 to December 2021 in our hospital,including 40 cases of pancreatic ductal adenocarcinoma,28 cases with benign tumors(13 cases of chronic pancreatitis,5 cases of lipoma,4 cases of cystadenoma and 6 cases of islet cell tumor).In addition,healthy volunteers were included in the control group for the same period.All subjects underwent 3.0T MR elastography,with a resonance frequency of 40 Hz.Results The average elasticity value of pancreatic cancer tissue was 4.33±1.69,the average elasticity value of pancreas benign place was 1.93±0.86,the normal pancreatic tissue was 1.13±1.10,the difference between the two groups was statistically significant(P<0.05);The elasticity value was more than 1.325 kPa in diagnosis of benign pancreatic ROC curve under the area of 0.748,the sensitivity was 71.4%,the specificity was 100%;If the elastic value was set for more than 3.310 kPa for the differential diagnosis of benign and malignant pancreatic lesions area of ROC curve was 0.871,the sensitivity was 75%,the specificity was 89.3%.Conclusion MRE can be used to measure tissue elasticity,effectively distinguish between normal pancreas and pancreatic lesions in the differential diagnosis of benign and malignant,with high clinical value,which is worthy of clinical application.
