低温等离子靶点消融治疗腰椎间盘突出症的临床价值分析

摘　　要：目的探讨弯头低温等离子靶点消融治疗腰椎间盘突出症的临床价值。方法采用透视引导弯头低温等离子靶点消融髓核成形术治疗腰椎间盘突出症患者37例,分别于术前,术后1周、3个月、6个月评估患者视觉模拟评分(visual analogue scale,VAS)和Oswestry功能障碍指数(oswestry disability index,ODI),采用改良的Macnab标准对术后6个月时手术疗效的优良率进行评价。结果术后1周、3个月、6个月的VAS评分和ODI指数均较术前明显降低(P<0.05),术后各时间点VAS评分和ODI指数差异无统计学意义(P>0.05),术后6个月疗效评价“优”19例(31.35%)、“良”12例(32.43%)、“一般”5例(13.52%)、“差”1例(2.70%),优良率为83.78%(31/37)。所有患者无严重并发症发生。结论弯头低温等离子靶点消融治疗腰椎间盘突出症操作安全,近期疗效显著。Objective To evaluate the clinical efficacy of the flexible coblation nucleoplasty for the targeted treatment of lumbar disc herniation.Methods 37 patients with lumbar disc herniation were treated with targeted coblation nucleoplasty under fluoroscopy guidance.The visual analogue scale(VAS)and the oswestry disability index(ODI)were assessed before and 3 weeeks,3months and 6 months after the operation respectively.The modified Macnab criterion was applied to evaluate the clinical efficacy at 6 months after operation.Results The VAS score and ODI at 1 week,3 months and 6 months after operation were significantly lower than those before operation(P<0.05),but there was no statistically significant difference between all post-treatment time points(P>0.05).All patients completed Macnab assessment till the 6 months followed-up after operation,and the surgical effect was excellent in 19 cases(51.35%),good in 12 cases(32.43%),fair in 5 cases(13.52%)and poor in 1 case(2.70%),the excellent and good rate of therapy was 83.78%(31/37).Conclusion The flexible coblation nucleoplasty can treat targetedly lumbar disc herniation,which is safe to operate,and the short-term effect is significant.
