摘　　要：目的观察5-氨基酮戊酸光动力(ALA-PDT)联合局部封闭治疗中重度痤疮的临床疗效。方法将98例中重度痤疮患者按就诊先后顺序随机分成联合组和ALA-PDT组,联合组49例,局部封闭预处理后予以ALA-PDT治疗;ALA-PDT组49例,直接进行ALA-PDT治疗,两个组均每2周治疗1次,治疗3次为1疗程。记录两组患者治疗前及治疗2、4、6、8、16周时疗效及不良反应情况。结果治疗4周时,联合组有效率(71.43%)高于ALA-PDT组(46.94%)(P<0.05);治疗6、8、16周时,联合组治愈率(61.22%、75.51%、87.76%)均高于ALA-PDT治疗组(38.78%、55.10%、69.39%)(P均<0.05)。结论ALA-PDT联合局部封闭及单纯ALA-PDT治疗中重度痤疮临床上均有效,但联合组起效更快,治愈率更高。Objective To observe the clinical efficacy of 5-aminolevulinic acid photodynamic therapy(ALA-PDT)combined with local blocking in the treatment of moderate to severe acne.Methods Ninety-eight patients with moderate to severe acne were randomly divided into two groups according to the order of presentation:the combined ALA-PDT group(n=49)treated with local blocking pretreatment followed by ALA-PDT every 2 weeks for 3 times as a course of treatment;the ALA-PDT group was directly treated with ALA-PDT every 2 weeks for 3 times as a course of treatment.In both groups,the efficacy was judged and adverse reactions were recorded on the 2nd,4th,6th,8th and 16th weeks of treatment.Results The effective rate of the combined group(71.43%)was significantly higher than that of the ALA-PDT group(46.94%)on the 4th weeks of treatment,and the difference was statistically significant(P<0.05);the cure rate of the combined group(61.22%,75.51%and 87.76%)was higher than that of the ALA-PDT group(38.78%,55.10%and 69.39%)on the 6th,8th and 16th weeks of treatment,and the differences were statistically significant(P<0.05).Conclusion Both ALA-PDT combined with local blocking and ALA-PDT alone have significant clinical efficacy in the treatment of moderate to severe acne,but the combined group has a faster onset of action,a cure rate,and has long-term cosmetic advantages,which is worthy of clinical application.
