摘　　要：目的评价重组结核分枝杆菌融合蛋白(EC)在学校结核病筛查中的应用效果,为学校结核病筛查试剂选择策略提供依据。方法通过诊断性试验方法,于2021-01对重庆市某区县1所学校发生肺结核疫情的251名学校密切接触者开展EC皮肤试验和体外γ-干扰素释放试验(interferon-gamma release assays,IGRA)检测(检测方法为QFT(QuantiFRRON-TB Gold In-Tube)),以IGRA为金标准,计算EC和IGRA检查的一致率和Kappa值。组间比较采用χ^(2)检验,检验水准α=0.05。结果在学校密切接触者潜伏结核感染筛查中,EC阳性率为9.17%,IGRA阳性率为11.54%,差异无统计学意义(χ^(2)=0.72,P=0.40);EC与IGRA一致率为95.59%,Kappa值0.76。结论EC与IGRA在学校结核病筛查中具有较好的一致性,可作为学校结核病筛查的候选试剂。Objective To evaluate the use of recombinant Mycobacterium tuberculosis fusion protein(EC)in screening for tuberculosis in schools,and to provide basis for selection strategy of tuberculosis screening reagents in schools.Methods In January 2021,By means of diagnostic tests,EC skin testing and in vitro interferon gamma release assay(IGRA,The detection method is QuantiFRRON-TB Gold In-Tube)testing was conducted on 251 close contacts of TB in a school in Chongqing.The consistency rate Kappa values of EC and IGRA were used to compare the differences between the two methods.Chi-square test was used for comparison betweer groups α=0.05.Results In the screening of latent tuberculosis infection in close contacts of school,the positive rate of EC testing was 9.17% and the positive rate of IGRA testing was 11.54%,with no statistically significant difference(χ^(2)=0.72,P=0.40);the consistency rate of EC and IGRA was 95.59% and a Kappa value of 0.76.Conclusion IGRA testing in campus TB screening and can be used as a candidate diagnostic reagent for campus TB screening.The EC testing has good concordance with.
