外科住院医师规范化培训教学查房流程的设计探讨

摘　　要：目的深入剖析当前外科住院医师规范化培训(简称住培)教学查房中存在的问题,设计外科住培教学查房流程。方法通过分析组织的外科住培教学查房活动,积极收集住院医师的反馈意见,结合相关的文献报道,查找外科住培教学查房设计中存在的问题,并针对这些问题设计外科住培教学查房流程。结果特此设计了"示教室-病房-示教室"三段式外科住培教学查房模式,贯彻住院医师为主体,教师主导的教学理念。参与人员需包括:患者、教师、助教、住院医师及护士。教学查房时间一般控制在80 min,病房内查房时间不少于35 min。住培基地秘书应提前1~2 d选择一个典型病例,并告知参与教学查房活动的住院医师。住院医师需提前做好准备工作。具体查房流程按照示教室介绍、病房查房和示教室讨论3个阶段进行,同时综合应用讲授法、直观演示和模拟教学法、启发式教学法等多种教学方法。结论该三段式外科住培教学查房模式能有效提升住院医师的疾病诊疗水平及临床思维能力,值得在各个住培基地大力推广。Objective To analyze the existing problems in the current surgical residential teaching rounds,and design a standardized surgical residential teaching rounds process.Method By analyzing our surgical residential teaching rounds,collecting feedback from residents,and analyzing related literature,problems were found in the design of surgical residential teaching rounds.And a standardized process was designed.Results This study led to a threestage surgical residential teaching rounds model of"classroom-ward-classroom",and the teaching concept of"residentcentered,teacher-led".Participants should include:patients,teachers,teaching assistants,residents and nurses.The teaching rounds were generally kept within 80 minutes,and the rounds in the ward were not less than 35 minutes.The secretary of the training base should select a typical case 1~2 days in advance and inform the residents who would participate in the teaching rounds.Residents needed to make preparations before the rounds.The specific rounds process consisted of three stages:introduction in the classroom,rounds in the ward,and discussions in the classroom.At the same time,a variety of teaching methods such as lecture method,visual demonstration and simulation teaching method,heuristic teaching method were comprehensively applied.Conclusion The three-stage surgical residential teaching rounds model can effectively improve residents’disease diagnosis and treatment level and clinical thinking ability,and it is worthy of vigorous promotion in various residential training bases.
