摘　　要：目的探究并分析一次性宫颈扩张器用于宫腔镜手术前扩张宫颈的临床疗效。方法选取2018年7月至2019年7月治疗的宫腔镜患者120例,按照随机数字表法将其分为观察组60例和对照组60例。对照组给予米索前列醇400μg软化宫颈,观察组给予一次性宫颈扩张器。观察并记录2组患者一般资料(包括年龄、BMI、疾病种类等)、宫颈扩张程度、患者VAS评分、感染指标(血清CRP及PCT)及不良反应。结果2组患者一般资料(包括年龄、BMI、疾病种类等),宫颈扩张程度比较差异有统计学意义(P<0.05),观察组患者宫颈扩张程度显著优于对照组,差异有统计学意义(P<0.05);经重复方差分析,2组患者VAS评分比较差异有统计学意义(F=11.24,P=0.000、F=8.27,P=0.016、F=10.24,P=0.004),术毕,2组患者VAS评分比较差异无统计学意义(P>0.05),术后6 h、12 h观察组患者VAS评分显著低于对照组,差异有统计学意义(P<0.05);2组患者手术前后血清CRP及PCT比较差异有统计学意义(P<0.05),术后2组患者血清CRP及PCT均显著升高,差异有统计学意义(P<0.05),术后6 h、12 h观察组患者血清CRP及PCT显著低于对照组,差异有统计学意义(P<0.05);2组患者不良反应比较差异有统计学意义(P<0.05),对照组患者不良反应平均分为(1.02±0.12)分,观察组患者不良反应平均分为(0.34±0.10)分,显著低于对照组,差异有统计学意义(P<0.05)。结论一次性宫颈扩张器能有效增加宫腔镜手术患者宫颈松弛度,改善术后VAS评分,减少术后感染等不良反应等发生,值得临床推广。Objective To investigate the clinical efficacy of disposable cervical dilator in expanding cervix before hysteroscopy.Methods A total of 120 patients who underwent hysteroscopy in our hospital from July 2018 to July 2019 were enrolled in the study,who were divided into control group and observation group,with 60 cases in eac hgroup.The patients in control group were given misoprostol 400μg to soften the cervix,however,the patients in observation group were examined by disposable cervical dilator.The general data(patient’s age,BMI,disease type),and cervical dilatation degree,VAS score,infection indexes(serum CRP and PCT)and adverse reaction score were observed and compared between the two groups.Results There were no significant differences in general data including age,BMI,disease type between the two groups(P>0.05),however the cervical dilatation degree in observation group was superior to that in control group(P<0.05).The repeated analysis of variance showed that there were significant differences in VAS scores before operation between the two groups(P<0.05),however,there were no significant differences in VAS scores at the end of operation between the two groups(P>0.05),moreover,which at 6h,12h after operation in observation group were significantly lower than those in control group(P<0.05).After operation,the serum levels of CRP and PCT in both groups were significantly increased(P<0.05),moreover,which at 6h,12h after operation in observation group were significantly lower than those in control group(P<0.05).In addition the average scores of adverse reactions in observation group were(0.34±0.10)points,which were significantly lower than those[(1.02±0.12)points]in control group(P<0.05).Conclusion The disposable cervical dilator can effectively increase the relaxation degree of cervix,improve the VAS scores and reduce the incidence of adverse reactions after operation in patients undergoing hysteroscopic surgery,therefor,which is worthy of clinical promotion.
