铋剂四联对胃溃疡患者血清炎性因子、H.pylori根治率及细胞免疫指标的影响

摘　　要：目的探讨铋剂四联对胃溃疡患者血清炎性因子、幽门螺杆菌(H.pylori)根治率以及细胞免疫指标的影响,旨在为临床利用铋剂四联提升胃溃疡治疗水平提供依据。方法收集2019年3月至2020年8月胃溃疡患者128例。按照随机数字表法分为研究组64例和对照组64例。对照组给予艾司奥美拉唑镁肠溶片联合阿莫西林胶囊和四环素片治疗,研究组给予铋剂四联(枸橼酸铋钾胶囊+艾司奥美拉唑镁肠溶片+阿莫西林胶囊+四环素片)治疗,比较2组患者的血清炎性因子、H.pylori根治率以及细胞免疫指标。结果2组患者血清炎性因子TNF-α、IL-2、IL-4、IL-6以及IL-10水平较治疗前均有显著改善,而且研究组患者较对照组患者的上述炎性因子指标改善更明显,差异有统计学意义(P均<0.05);研究组的H.pylori根治率显著高于对照组(χ^(2)=4.2,P=0.04);治疗后研究组细胞免疫指标CD_(3)^(+)、CD_(4)^(+)、CD_(8)^(+)、CD_(4)^(+)/CD_(8)^(+)均明显优于对照组,差异有统计学意义(P均<0.05);2组患者不良反应发生率接近,差异无统计学意义(P>0.05)。结论铋剂四联治疗胃溃疡患者效果显著,可有效改善患者血清中炎性因子水平,有效抑制患者体内炎性反应,提高H.pylori根治率,有效改善患者免疫功能,且治疗安全性较高,此治疗方案值得在临床上应用和推广。Objective To investigate the effects of bismuth quad on the serum inflammatory factors,radical cure rate of H.pylori and cellular immune indexes in patients with gastric ulcer,in order to provide basis for improving the curative effects on gastric ulcer.Methods A total of 128 patients with gastric ulcer who were treated in our hospital from June 2018 to September 2019 were enrooled in the study,who were randomly divided into control group and research group,with 64 cases in each group.The patients in control group were treated by esomeprazole magnesium enteric-coated tablets combined with amoxicillin capsules and tetracycline tablets,however,thepatients in research group were treated by bismuth quad Bismuth potassium citrate capsules+esomeprazole magnesium enteric-coated tablets+amoxicillin capsules+tetracycline tablets.The changes of inflammatory cytokines,and H.pylori eradicate rate as well as cellular immunity indexes were observed and compared between the two groups.Results After treatment,the levels of TNF-α,IL-2,IL-4,IL-6 and IL-10 in both groups were significantly improved,moreover the improvement degree in research group was more significant than that in control group(P<0.05).The radical cure rate of H.pylori in research group was significantly higher than that in control group(P<0.05).Moreover the levels of CD_(3)^(+),CD_(4)^(+),CD_(8)^(+) and CD_(4)^(+)/CD_(8)^(+) in research group were superior to those in control group(P<0.05).However there was no significant difference in the incidence of adverse reactions between the two groups(P>0.05).Conclusion The curative effects of bismuth quad on gastric ulcer are significant,which can effectively improve the serum levels of inflammatory factors,effectively inhibit the inflammatory response of patients,improve the radical cure rate of H.pylori,effectively improve the immune function of patients,with better safety,therefoe,it is worthy of clinical application and promotion.
