河北省适龄女性宫颈HPV感染现状及配偶感染情况分析

摘　　要：目的了解河北省适龄女性宫颈人乳头状瘤病毒(HPV)的感染现状,感染者配偶外生殖器HPV感染情况,以及男女双方HPV亚型吻合情况。方法以在河北省直属机关门诊接受宫颈HPV检查的女性5000例为研究对象,检查结果阳性者配偶行外生殖器上皮细胞性HPV检测,并对结果进行分析。结果在5000例适龄女性中HPV共检测出阳性802例。总HPV感染率为16.04%(802/5000),其中高危型和低危型HPV感染率分别为14.22%(711/5000)和1.82%(91/5000);HPV单一感染和多重感染率分别为11.88%(594/5000)和2.9%(145/5000);HPV单一感染前10位型别是HPV16、52、58、33、51、53、低危81、39、18、31型;多重感染以HR-HPV合并其他高危型为主,其中HPV16型合并其他高危型占首位。802例阳性女性中,配偶行HPV检测643例,男性HPV感染阳性199例,其中HPV亚型完全吻合67对,完全吻合率为33.67%(67/199)。结论河北省适龄女性宫颈病变人群中HPV感染主要以单一型别、高危型感染为主,未发现高危45亚型感染,<21岁年轻女性感染率最高,感染者配偶HPV阳性率为30.95%,配偶双方均感染HPV者其HPV亚型吻合度高,分别是高危亚型16、18,低危亚型6。Objective To investigate the current situation of cervical human papillomavirus(HPV)infection in age-appropriate women and their spouse in Hebei province,and to explore the coincidence status of HPV subtypes between men and women.Methods A total of 5,000 women who underwent cervical HPV examination in our center were enrolled in the study,and their spouses with positive results were tested for sexual HPV of external genital epithelial cells,and 20 high-risk and low-risk subtypes were detected.The results were analyzed.Results Of the 5,000 women,a total of 802 cases with positive HPV were detected,with the total HPV infection rate being 16.04%(802/5,000),of whom,the infection rates HPV high-risk type and low-risk type were 14.22%(711/5,000)and 1.82%(91/5,000),respectively,and HPV single infection and multiple infection rates were 11.88%(594/5,000)and 2.9%(145/5,000),respectively.The top 10 types of single infection of HPV were HPV16,52,58,33,51,53,low risk type 81,39,18,31.The multiple infection was mainly HR-HPV combined with the other high-risk types.Of the 802 women with positive HPV,their spouses(643 men)were detected,and 199 men were positive for HPV,there were 67 couples with complete coincidence of HPV subtypes,with complete coincidence rate being 33.67%(67/199).Conclusion The HPV infection is mainly single type and high-risk type infection in age-appropriate women with cervix lesion in Hebei province,and no high-risk 45 subtype infection is found,and infection rate is the highest in the females under 21 years of age,and positive rate of HPV in their spouses is 30.95%,and the HPV subtype anastomosis rate is higher in the couples being positive,and the subtypes are high-risk 16,18 types,low-risk 6 type.
