古塞奇尤单抗和司库奇尤单抗治疗中、重度银屑病的成本-效果比较

摘　　要：目的比较古塞奇尤单抗与司库奇尤单抗对中、重度银屑病的治疗成本-效果,为临床合理用药提供参考。方法以国内医疗成本为成本数据,以头对头临床试验结果为疗效指标,对比古塞奇尤单抗和司库奇尤单抗的成本-效果,并进行敏感度分析。结果以银屑病面积和严重程度指数(PASI)90为终点指标,古塞其尤单抗的成本-效果比为132701.4元,高于司库奇尤单抗的57702.9元。增量成本-效果比(ICER)为4972.8元,即每获得1%的有效率,需多支出4972.8元,远远低于意愿支付阈值(WTP,两种药物平均总成本为76196元),说明使用古塞奇尤单抗的成本-效果性更佳。敏感性分析显示本研究结果稳定可靠。结论在中、重度银屑病患者的治疗中,以PASI 90为终点指标,古塞奇尤疗效优于司库奇尤,以两种方案的平均成本作为意愿支付阈值,古塞奇尤单抗具有一定成本-效果优势。Objective To compare the cost-effectiveness of gusekimab and skuzimab in patients with moderate and severe psoriasis,so as to provide reference for clinical rational drug use.Methods Taking the domestic medical cost as the cost data and the results of head-to-head clinical trials as the efficacy index,the cost-effectiveness of guseguizumab and scueguizumab were compared,and the sensitivity was analyzed.Results Taking the psoriasis area and severity index(PASI)90 as the endpoint index,the cost-effectiveness ratio of gusetizumab was 132701.4 yuan,which was higher than 57702.9 yuan of scuchizumab.The incremental cost-effectiveness ratio(ICER)was 4972.8 yuan,that was,for every 1%effective rate,an additional expenditure of 4972.8 yuan was required,which was far lower than the willingness to pay threshold(WTP,the average total cost of the two drugs is 76196 yuan),indicating that the cost-effectiveness of using guseguizumab was better.Sensitivity analysis showed that the results of this study were stable and reliable.Conclusion In the treatment of patients with moderate and severe psoriasis,taking PASI 90 as the endpoint index,the curative effect of gusekiyu is better than that of skukiyu.Taking the average cost of the two schemes as the willingness to pay threshold,gusekiyu has a certain cost-effectiveness advantage.
