摘　　要：目的探讨非瓣膜性房颤患者心力衰竭发生的影响因素。方法选取2017年7月至2021年7月商丘市第一人民医院收治的106例非瓣膜性房颤患者作为研究对象,所有患者入院时接受超声心动图检查,参照相关标准评估非瓣膜性房颤患者心力衰竭发生情况;记录患者基线资料,分析非瓣膜性房颤患者心力衰竭发生的影响因素。结果106例非瓣膜性房颤患者经超声心动图检查,确诊心力衰竭32例(30.19%),未发生心力衰竭74例(69.81%);发生组入院时肿瘤坏死因子-α(TNF-α)、同型半胱氨酸(Hcy)、血清游离甲状腺素(FT_(4))水平高于未发生组(P<0.05);经logistic回归分析,结果显示,入院时TNF-α、Hcy、FT_(4)过表达是非瓣膜性房颤患者发生心力衰竭的影响因素(OR>1,P<0.05)。结论TNF-α、Hcy、FT_(4)过表达是非瓣膜性房颤患者发生心力衰竭的影响因素。Objective To explore the influencing factors of heart failure in patients with non-valvular atrial fibrillation.Methods A total of 106 patients with non-valvular atrial fibrillation admitted to Shangqiu First People’s Hospital from July 2017 to July 2021 were selected as the research subjects,all patients underwent echocardiography at admission.The occurrence of heart failure in patients with non-valvular atrial fibrillation was evaluated according to relevant standards,and the baseline data of patients were recorded.The influencing factors of heart failure in patients with non-valvular atrial fibrillation were analyzed.Results In 106 patients with non-valvular atrial fibrillation,32 cases(30.19%)were diagnosed as heart failure by echocardiography,and 74 cases(69.81%)did not have heart failure.The levels of tumor necrosis factor-α(TNF-α),homocysteine(Hcy)and serum free thyroxine(FT_(4))at admission in the occurrence group were higher than those in the non-occurring group(P<0.05).Through logistic regression analysis,the results showed that the overexpression of TNF-α,Hcy and FT_(4) at admission was the influencing factor of heart failure in patients with non-valvular atrial fibrillation(OR>1,P<0.05).Conclusion The overexpression of TNF-α,Hcy and FT_(4) is the influencing factor of heart failure in patients with non-valvular atrial fibrillation.
