摘　　要：目的探究Fibro Touch参数与慢性乙型病毒性肝炎(CHB)患者血糖及胰岛素水平的相关性。方法选取2019年4月至2020年10月期间东莞市厚街医院收治的168例CHB患者作为研究对象,按照肝纤维化程度分为F0期19例,F1期29例,F2期40例,F3期51例,F4期29例。采用Fibro Touch检查测定其肝硬度测定值(LSM),比较不同肝纤维化分期CHB患者的谷丙转氨酶(ALT)、总胆红素(TBIL)、LSM、糖化血红蛋白(HbAlc)和胰岛素水平,采用Spearman相关性分析LSM、HbAlc和胰岛素之间的相关性。结果随着肝纤维分期的不断增加,ALT、TBIL、LSM、HbA1c水平呈现逐渐上升的趋势,胰岛素水平呈现逐渐下降的趋势,差异均有统计学意义(P<0.05);CHB患者的LSM水平与HbA1c水平呈正相关(r=0.382,P<0.01),LSM水平与胰岛素水平、HbAlc水平与胰岛素水平均呈负相关(r=-0.412、-0.336,P<0.01)。结论Fibro Touch检查测定CHB患者的LSM水平与其血糖和胰岛素水平具有相关性。Fibro Touch检查与血糖、胰岛素水平在评估CHB患者肝纤维化程度上具有一定的临床应用价值。Objective To explore the correlation between Fibro Touch parameters and the levels of blood glucose and insulin in patients with chronic hepatitis B(CHB).Methods The 168 CHB patients admitted to Dongguan Houjie Hospital from April 2019 to October 2020 were selected as the research objects.According to the degree of liver fibrosis,there were 19 cases in F0 stage,29 cases in F1 stage,40 cases in F2 stage,51 cases in F3 stage,and 29 cases in F4 stage.Fibro Touch test was used to measure the liver stiffness measure(LSM)values.The levels of alanine aminotransferase(ALT),total bilirubin(TBIL),LSM,glycosylated hemoglobin(HbAlc)and insulin were compared in CHB patients with different stages of liver fibrosis.Spearman correlation was used to analyze the correlation between LSM,HbAlc and insulin.Results With the continuous increase of hepatic fiber staging,the levels of ALT,TBIL,LSM and HbA1 c increased gradually,and the level of insulin decreased gradually(P<0.05).The LSM level was positively correlated with HbA1 c level in CHB patients(r=0.382,P<0.01),LSM level was negatively correlated with insulin level,and HbAlc level was negatively correlated with insulin level(r=-0.412,-0.336,P<0.01).Conclusion The Fibro Touch determines that the LSM level of CHB patients is correlated with their blood glucose and insulin levels.Fibro Touch examination and blood glucose and insulin levels have certain clinical application value in evaluating the degree of liver fibrosis in patients with CHB.
