X线摄影与超声检查对婴幼儿发育性髋关节发育不良的诊断价值

摘　　要：目的比较X线摄影与超声检查在婴儿发育性髋关节发育不良(DDH)中的诊断价值,为婴幼儿DDH的临床诊断方法的选择提供依据。方法选择宝鸡市妇幼保健院于2017年1月至2020年12月期间临床确诊为DDH的45例患儿为研究对象,其中有56个髋关节发育不良,34个髋关节发育正常,均接受X线及超声检查,比较两种检查方法的诊断价值,并使用Kappa检验两种方法的一致性。结果 56个DDH髋关节中,X线检查诊断符合者41个,而超声检查诊断符合者48个,超声检查诊断DDH的符合率为85.71%,略高于X线检查的73.21%,但两者比较差异无统计学意义(P>0.05);对X线和超声检查结果用Kappa检验比较,Kappa值为0.782,两种检查结果基本一致。结论 X线检查与超声检查均是婴幼儿DDH的有效诊断方法,两者检查结果基本一致,临床上可根据实际情况对检查方法进行合理选择。Objective To compare the diagnostic value of X-ray photography and ultrasonography in infants with developmental dysplasia of the hip(DDH),so as to provide a basis for the selection of clinical diagnostic methods for infant DDH.Methods A total of 45 children were selected as the research objects,who were clinically diagnosed as DDH in the Baoji Maternal and Child Health Care Hospital from January 2017 to December 2020.Among them,56 had hip dysplasia and 34 had normal hip development.All of them received X-ray photography and ultrasonography.The diagnostic value of the two methods were compared,and Kappa was used to test the consistency of the two methods.Results Among the 56 cases of DDH hips,41 hips were diagnosed by X-ray,and 48 hips were diagnosed by ultrasound.The coincidence rate of DDH diagnosis by ultrasound(85.71%) was slightly higher than that of X-ray(73.21%);but there was no statistically significant difference between the two groups(P>0.05).Kappa test was used to compare the results of X-ray photography and ultrasonography,the Kappa value was 0.782,and the results of the two examinations were basically the same.Conclusion X-ray and ultrasound examination are both effective methods for diagnosing infant DDH.The examination results of the two methods are basically the same.In clinic,the examination method can be reasonably selected according to the actual situation.
