摘　　要：目的:探究股骨近端防旋髓内钉(proximal femoral nail antirotation,PFNA)内固定在股骨粗隆间骨折患者中的应用效果。方法:选取2019年1月-2021年1月本院收治的股骨粗隆间骨折患者80例。通过随机数字表法分为研究组(n=40)和对照组(n=40),两组一般资料比较差异无统计学意义(P>0.05)。对照组实施股骨近端外侧锁骨钢板内固定,研究组实施PFNA内固定。比较两组手术时长、术中失血量、术后愈合时间、术后并发症发生率及术前、术后6个月Harris评分。结果:研究组有效率明显高于对照组(P<0.05),研究组手术时长(61.11±3.61)min、术中失血量(180.23±20.23)ml及术后愈合时间(3.45±1.14)个月,均明显优于对照组的(72.23±5.44)min、(246.14±40.99)ml、(4.15±0.13)个月(t=-10.772、-9.119、-3.858,P<0.05);研究组术后并发症发生率(7.50%)低于对照组(30.00%)(χ^(2)=5.251,P=0.021);研究组术后6个月的Harris评分为(90.12±2.22)分,高于对照组的(83.23±3.16)分(t=11.284,P<0.05)。结论:PFNA内固定在治疗股骨粗隆间骨折患者中具有微创、手术时间短、术中失血量少、术后并发症发生率低及恢复迅速等优点,值得进行临床普及推广。Objective:To explore the application effect of proximal femoral nail antirotation(PFNA)internal fixation in patients with femoral intertrochanteric fracture.Method:A total of 80 patients with femoral intertrochanteric fractures admitted to our hospital from January 2019 to January 2021 were selected and divided into the study group(n=40)and the control group(n=40)according to the random number table method.There was no significant difference in general information between the two groups(P>0.05).The control group received internal fixation with the lateral clavicle plate of the proximal femur,the study group received PFNA internal fixation.The operation time,intraoperative blood loss,postoperative healing time,the incidence of postoperative complications and Harris scores before and 6 months after operation were compared between the two groups.Result:The effective rate of the study group was higer than that of the control group(P<0.05),the operation time(61.11±3.61)min,intraoperative blood loss(180.23±20.23)ml,and postoperative healing time(3.45±1.14)months in the study group were significantly better than(72.23±5.44)min,(246.14±40.99)ml,(4.15±0.13)months in the control group(t=-10.772,-9.119,-3.858,P<0.05);the incidence of postoperative complications in the study group(7.50%)was lower than that in the control group(30.00%)(χ^(2)=5.251,P=0.021);the Harris score(90.12±2.22)points of the study group was higher than that of the control group(83.23±3.16)points at 6 months after surgery(t=11.284,P<0.05).Conclusion:PFNA internal fixation has the advantages of minimally invasive,short operative time,low intraoperative blood loss,low incidence of postoperative complications and rapid recovery in the treatment of patients with femoral intertrochanteric fracture.It is worthy of clinical popularization and promotion.
