RPH术与TST术加外剥内扎术治疗混合痔的临床比较

摘　　要：目的:比较自动痔疮套扎术(RPH)与选择性痔上黏膜切除术(TST)术加外剥内扎术治疗混合痔的应用效果。方法:选择笔者所在医院2020年1月-2021年1月收治的120例混合痔患者,采用随机数字表法将其分为观察组及对照组,各60例。观察组行RPH术加外剥内扎术,对照组行TST术加外剥内扎术。比较两组手术时间、术中出血、术后疼痛、住院时间及并发症情况。结果:观察组手术时间、术中出血量、住院时间均优于对照组(P<0.05),对比术后疼痛及愈合时间,观察组优于对照组(P<0.05);对照组术后发生出血、水肿、肛门坠胀和尿潴留均多于观察组(P<0.05);两组总有效率比较,差异无统计学意义(P>0.05)。结论:RPH术加外剥内扎术治疗混合痔手术时间短,可以减少术中创面出血,减轻术后切口疼痛,并且能够减少并发症发生,加快术后创口愈合。Objective:To compare the effects of ruiyun procedure for hemorrhoids(RPH)and tissue-selecting therapy stapler(TST)combined with external stripping and internal ligation in the treatment of mixed hemorrhoids.Method:A total of 120 patients with mixed hemorrhoids treated in our hospital from January 2020 to January 2021 were randomly divided into observation group and control group according to random table method,with 60 cases in each group.The observation group received RPH plus external stripping and internal ligation,and the control group received TST plus external stripping and internal ligation.The operation time,intraoperative bleeding,postoperative pain,hospital stay and complications were compared between the two groups.Result:The operation time,intraoperative bleeding volume and hospital stay in the observation group were better than those in the control group(P<0.05);compared the postoperative pain and healing time,the observation group was better than the control group(P<0.05).Postoperative bleeding,edema,anal distention and urinary retention in the control group were more than those in the observation group(P<0.05).There was no significant difference in the total effective rate between the two groups(P>0.05).Conclusion:RPH combined with external stripping and internal ligation can reduce intraoperative wound bleeding,reduce postoperative incision pain,reduce complications and accelerate postoperative wound healing.
