莆田市家庭医生签约服务及影响因素研究

摘　　要：目的了解莆田市全科医生基本情况及居民家庭医生服务签约现状,探讨影响家庭医生签约服务的相关因素。方法在莆田市随机抽取434名全科医生及457名居民,采用自制调查问卷对全科医生基本情况及居民家庭医生服务签约现状进行调查,采用单因素和多因素Logistic回归分析影响家庭医生签约服务的相关因素。结果被调查的全科医生中工作单位主要为城市社区卫生服务机构(62.4%);多因素Logistic回归分析显示居民是否知晓家庭医生签约服务、是否知晓分级诊疗、上级医院转诊基层医疗机构的经历、家庭医生签约服务满意度、居住地至社区卫生服务机构步行时间、是否患有慢性病是影响居民签约的影响因素。结论全科医生存在职称及注册率低,高层次学历人数较少,城乡分布不均情况,建议加强全科医生培训、大力培养全科医生队伍,向居民推广宣传家庭医生服务签约益处以促进基层医疗服务开展。Objective To learn about the basic situation offamily physician in Putian City and the current status of contracted services of family physicians for residents,and to explore the factors that affect the contracted services of family physicians.Methods 434 general practitioners and 457 residents in Putian City were randomly selected.Self-made questionnaires were used to investigate the basic situation of general practitioners and the status of contracted services of family physicians for residents.Single-factor and multiple logistic regression analysis was used to analyze the related influencing factors of status of contracted services of family physicians.Results The main work units of the surveyed family physicians were urban community health service institutions(62.4%).Multiple logistic regression analysis showed the influencing factors that influenced residents to sign the family physicians service contracts included whether residents were aware of contracted services of family physicians,whether they were aware of hierarchical diagnosis and treatment,whether they had the experience of referral to primary care from higher-level hospitals,their satisfaction of the contracted service of family physicians,the walking time from the place of residence to the community health center,and the presence of chronic diseases.Conclusion Family physicians have a low professional title and registration rate.The family physicians with high-level academic qualifications account for poor proportion.There is uneven distribution between urban and rural areas.It is recommended to strengthen family physician training,vigorously cultivate the team of family physicians,and promote the benefits of signing family physicians services to residents to promote the development of grassroots medical service.
