托法替布治疗顽固性瘙痒的斑块状银屑病1例

摘　　要：患者女,59岁,全身散在红色丘疹、斑块、鳞屑伴剧烈瘙痒4年余,既往接受甲氨蝶呤等治疗,效果欠佳。皮肤科情况:双侧小腿伸侧、肘部及骶尾部见散在红色丘疹、斑块,其上附着鳞屑、痂壳。瘙痒严重程度评分:9.50分。皮损组织病理示:表皮角化过度伴角化不全,棘层增生,真皮浅中层淋巴细胞浸润。诊断:斑块状银屑病。治疗方案:托法替布5 mg, 2次/d。随访12周瘙痒明显减轻,皮损清除,无不良事件报告。A 59-year-old female presented with red papules and plaques covered with scales scattered around the body for over 4 years, complaining about intense itching.Medications including Methotrexate did not achieve a good therapeutic effect.Physical examination showed red papules and plaques covered with scales and blood crusts scattered on the extension part of the calves, elbows, and lumbosacral portion.Itch severity score: 9.50.Histopathological findings: hyperkeratosis, parakeratosis, acanthosis in the epidermis, and infiltration of lymphocytes from the superficial to the middle dermis.The patient was diagnosed with plaque psoriasis, and was prescribed Tofacitinib, 5 mg, 2 times per day.During the 12-week follow-up, the lesions and itchiness were significantly relieved, without adverse effects.
